.o ** INVALID FAILURE OF DIESEL GENERATOR *A' DUE TO OPERATOR ERROR IN LOADING THE GENERATOR WITHIN ALLOWED TIME LIMITS

.On 7/18/90, at 1025 CDT, Emergency Diesel Generator (D/C) 'A' failed to e start during performance of a surveillance procedure due to operator error. l This operator error is attributable to a procedural inadequacy. This was an invalid failure which is reported per Technical Specification-(T/S) 4.8.1.1.3.

Procedure OSP-NE 00002, Standby Diesel Generator Periodic Tests, step 6.3.6 requires paralleling of the generator with the bus in accordance with

, OTN NE.00001, Standby Diesel Generation System, step 4.2.3. This step ensures that the synchroscope is moving in the fast direction and sets a general criteria for the rate at which the synchroscope should be moving.

The procedure then instructs the operator to immediately increase the diesel speed if the load is not 200 KW. A teverse power relay is set to trip the breaker if the generator does not see 109 KW of load within three seconds. On an emergency start, the reverse power relay protection is defeated.

  • Per the test procedure, a non. licensed utility operator trainee, acting under the direct supervision of a licenaed utility operator, paralleled the generator with the bus, but failed to lead the generator within the three seconds allowed by the reverse power relay. Therefore, the generator breaker tripped after receiving a reverse power relay actuation.

After consultation with the system engineer, the reverse power relay was reset and the generator was resynchronized to the bus and loaded successfully. The D/G 'A' surveillance OSP.NE.00002 was completed. ,

OTN.NE.00001 has been revised to more clearly reflect the loading requirements when paralleling the D/G, The D/G 'A' availability to perform its safety function was not affected by this event. Since the failure is directly attributable to operator error, this was an invalid failure and the surveillance test interval of T/S Table ,

4.81 is not affected. This position is consistent with Regulatory Guide 1.108 C.2.e(2).

  1. Special Reports 85-01, 85-02, 85 07, 86 01, 89 02, 89 03, 89 07 Surveillance tests are currently performed at least once per 31 days for D/G 'A'. This is in conformance with the schedule in T/S Table 4.8 1 which requires a test interval of not more than 31 days if the number of failures in the last 20 valid tests is one or less, or the number of failures in the last 100 valid tests is four or less.
