i SPECIAL REPORT 89-09 LOOSE-PART DETECTION SYSTEM CHANNEL INOPERABLE FOR GREATER THAN 30 DAYS This report is submitted in accordance with Technical Specification 3.3.3.8 Action (a) which states, "With one or more Loose-Part Detection System channel (s) inoperable for more than 30 days, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next 10 days outlining the cause of the malfunction and the plans for restor-ing the channel (s)'to OPERABLE status."
At 0022 CDT on 6/19/89, following a Main Control Board Loose-Part

annunciator and performance of.a generic work, request by utility Instrument and Controls (I&C) technicians, Channel 3 of the Loose-Part Detection System was determined to be inoperable. The plant was in Mode 1 at 100% power.
The Channel 3 accelerometer is located in the containment mounted on the reactor vessel head assembly. Initial troubleshooting of Channel 3 has determined that the problem lies with the sensor, sensor mounting or associated sensor cabling. Since the instrument sensor is connected to the reactor vessel head (thus inaccessible during power operations) further troubleshooting will not be performed until the next outage of sufficient duration. The remaining eleven channels in this system remain operable.
Until further troubleshooting has been completed, actions to be taken to prevent recurrence cannot be determined.
