SPECIAL REPORT 89-02 INVALID DIESEL GENERATOR 'A' FAILURE DUE TO PROCEDURAL INADEQUACIES WHICH LEFT SPEED CONTROL KNOB SET TOO HIGH TO PREVENT OVERSPEED TRIP on 3/29/89, at 1430 CST, emergency diesel generator (D/G) 'A' tripped on overspeed while being started for a post maintenance and fuel system priming run.

This was an invalid failure which is reported per Technical Specification (T/S) 4.8.1.1.3.

Investigation revealed the speed knob on the D/G governor was not set low enough to allow the mechanical governor to catch the D/G before it reached the overspeed trip. The speed knob had been turned down two times per plant procedure OTN-NE-00001, " Standby Diesel Generation System".

However, combined with the amount of fuel the D/G received on the start signal, the two turns were not sufficient to prevent the D/G from exceeding the overspeed trip setpoint.

The speed knob was turned down an additional eight to ten turns and the D/G was successfully restarted at approximately 1515. Procedure OTN-NE-00001 will be revised to require the speed knob to be rotated ten to 12 turns, then raised to normal before restoring the electric governor.

Prior to this event, the D/G had been declared inoperable at 0420 on 3/29/89 to permit maintenance on the fuel o!! transfer pump and the ventilation supply suction return air damper. The D/G 'A' was declared operable at 1743 on 3/29/89. The D/G was unavailable for 13 hours1.50463e-4 days <br />0.00361 hours <br />2.149471e-5 weeks <br />4.9465e-6 months <br /> and 23 minutes. T/S 3.8.1.1 action statements were satisfied.

Since this failure occurred during a maintenance run and not during a valid test, the surveillance test interval of T/S Table 4.8-1 is not affected. This position is consistent with Regulatory Guide 1.108.C.2.e(7).

This is in conformance with the schedule in T/S Table 4.8-1 which requires a test intervaJ of not more than 31 days if the number of failures in the last 20 valid tests is one or less or the number of failures in the last 100 valid tests is four or less.
