SPECIAL REPORT 89-07 INVALID FAILURE OF DIESEL GENERATOR 'A' DUE TO FAILED DRIVER RELAY CARD IN THE LOAD SHEDDING AND EMERGENCY LOAD SEQUENCING LOGIC PANEL On 4/1/89, at 1955 CST, emergency diesel generator (D/G) 'A' failed to start as requ1 red during performance of surveillance test procedure ISP-SA-2413A. " Diesel Generator and Sequencer Testing (Train A)", step 7.1, i " Blackout without Safety Injection Signal." This was an invalid failure which is reported por Technical ~ specification (T/S) 4.8.1.1.3. Investigation revealed that a relay driver card located in the load shedding and emergency load sequencing (LSELS) logic channel panel had failed. This relay driver card processes the 2/4 UV (loss of voltage) signal to supply a diesel start signal via LSELS. With this failure, the D/G did not receive a start signal with the planned loss of power to bus NB01. The defective relay driver card was replaced and proper operation of' LSELS was verified. A review of this particular card type failure was performed and increased periodic verification of proper relay driver operation will be performed. The train 'A' LSELS system was declared inoperable at 2000 on 4/1/89 and was declared operable at 0400 on 4/2/89. T/S 3.8.1.1 action statements were satisfied. Since the failed driver card was a component in the LSELS control, protection, and surveillance system and is not considered a part of the defined D/G unit design (ref: IEEE Standard 387-1977), this was an invalid failure. The. surveillance test interval of T/S Table 4.8-1 is not affected. This position is consistent with Regulatory Guide 1.108.C.2.e(2). 'A'. This is in conformance with the schedule in T/S Table 4.8-1 which requires a test interval of not more than 31 days if the number of failures in the last 20 valid tests is one or less or the number of failures in the last 100 valid tests is four or less. Page 1 of 2
SPECIAL REPORT 89-07 INVALID FAILURE OF' DIESEL GENERATOR 'A' DUE TO ~ FAILED DRIVER RELAY CARD IN THE LOAD SHEDDING AND EMERGENCY LOAD SEQUENCING LOAD PANEL t Submittal of this report was delayed due to an earlier utility interpre-tation that a failure of the driver relay card while not being a part of the defined D/G unit, was not reportable as a D/G failure. The Regulatory Guide 1.108.c.2.e(2) statement, " malfunction of equipment that. is not part of j the defined diesel generator unit design should not be considered valid tests or failurts," was interpreted to mean this condition was'not a valid test nor 1 was it a failure. Applying the adjective " valid" also to the word " failure" in the roove phrase changes the interpretation somewhat. Subsequently, it was determined on 7/17/89 that the failed start attempt was an invalid failuro as defined by Regulatory Guide 1.108.
