SPECIAL REPORT 86-05 LOOSE-PART DETECTION SYSTEM This report is being submitted in accordance with Technical Specification 3.3.3.8 ACTION (a) which states, "With one or more Loose-Part Detection System channels inoperable for more than 30 days, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next 10 days outlining the cause of the malfunction and the plans for restoring the channel (s) to OPERABLE status."
At approximately 0001 CDT on 7/8/86, during the Control Room Shif t and Daily Log Readings and Channel Checks Surveillance, Channel 5 of the Loose-Part Detection System was logged in the alarm status and declared inoperable. The plant was in Mode 1 at 100% power.
The Channel 5 accelerometer is located on Steam Generator "A" on the lower supporting flange. An additional accelerometer, Channel 6, is also located on Steam Generator "A" on the lower supporting flange, approximately 120* counter-clockwise from Channel 5.
Channel 6 was verified operable at the time of discovery.
Upon further investigation, it was found that the Steam Generator "A" Sludge Lance Hand Hole Cover was leaking onto the Channel 5 Loose-Part Detection System Hard Wire / Soft Wire Junction Box.
This created a ground condition, thus making Channel 5 inoperable. On 8/8/86, Channel 5 of the Looge-Part Detection System was inoperable for more than 30 days.
To correct the inoperability, work has been completed-to temporarily reseal the Hand Hole Cover to prevent further leakage onto the Channel 5 Hard Wire / Soft Wire Junction Box.
Work has been planned to permanently repair the Hand Hole Cover and the damage to the Channel 5 Junction Box.
Due to plant conditions (high radiation levels during plant power operations) at the location of the equipment to be repaired, further work will not be attempted until the first outage of sufficient duration, but not later than the' restart following the next refueling.
