SPECIAL REPORT 87-07 INOPERABLE SEISMIC MONITORING INSTRUMENTATION This report is being generated in accordance with Technical Specification (T/S) 3.3.3.3 Action (a) which states "with one or more of the above seismic monitoring instruments inoperable for more than 30 days, prepare and submit a Special Report to the Ccmmission pursuant to Specification 6.9.2 within the neAc 10 days outliatng the cause of the malfunction and the plans for restoring the instrument (s) to OPERABLE status."

On 6/1/87, Instrument and Controls (I&C) technicians were performing an 18-month channel calibration on the Containment Base Slab Triaxial Response ~ Spectrum Recorder (Passive) in accordence with T/S 4.3.3.3.2, Table 3.3-7, item 3.a.

During restoration frem tia procedure, the " scratch pads" for the 48 sensors (16 sensors ou each of three axes) were being re-inserted when the scriber tip for sensor #15 l

(vertical axis) was inadvertently broken. The Containment Base Slab recorder was declared inoperable at 1100 CDT on 6/1/87 thus placing the plant in T/S 3.3.3.3 Action (a). The p2 ant was in Mode 1 - Power Operation at 100% power at the time of this event.

A Work Request was initiated to replace the scriber on 6/1/87.

During replacement work on 6/12/87, further damage to sensor #15 was j

discovered. The intact portion of its scriber had separated from the base support which would have prevented the scriber tip from etching the

" scratch pad".

The cause of this damage is unknown and deemed to have l

occurred at the time of discovery with no firm evidence to believe otherwise..The other sensors were inspected at this time with no other problems found.

Sensor #15 will be replaced and is pending receipt of a new sensor j

on site. -Expected date of return to OPERABLE status is July 30, 1987.
