The Channel 5 accelerometer.is located on Steam Generator "A" on the lower supporting flange. .An additional accelerometer, Channel ~6, is

also located on Steam' Generator "A" on the lower supporting flange, L approximately 120' counter-clockwise from Channel 5. Channel 6 has been operable since Channel 5.was declared inoperable on 9/22/86. The plant
- has been.in Mode 1, Power Operation, since 9/22/86.
.Cause of the Malfunction-t l: Special Report 86-05 was submitted cni 8/15/86 to report the inoperability of Channel 5 for more than 30 days. The cause of this inoperability was'a ground condition in the Channel 5 Hard Wire / Soft l Wire Junction Box due to leaka'ge from Steam Generator "A" Sludge Lance P Mand Hole Cover. The Hand Hole Cover was temporarily resealed, the Junction Box cleaned, and Channel 5 restored to operability. Work requests were written to permanently repair the Hand' Hole Cover and l correct the damage to the Junction Box during the next outage of
! sufficient duration.

!- Channel 5 was again declared inoperable'at 0029 CDT on 9/22/86'due
- i. to the channel being in a vibration alarm state. Recurring leakage from E the Hand Hole Cover had created another ground condition in the Channel l
5 Junction Box. The channel presently remains inoperable due to the
! ground condition.
I Plans for Restoring the Channel to Operable Status
- f. The previously written work requests to repair the Hand Hole' Cover i and correct the damage to the Junction Box will restore the channel to operable status. Current plant conditions restrict completion of these work requests. The work will be performed during the next outage of sufficient duration, but not later than the restart following the next l' refueling outage. No additional Special Reports will be submitted on f the inoperability of Channel 5 of the Loose-Part' Detection System until the Hand Hole Cover and Junction Box are repaired during the next outage of sufficient duration.
