Tinnassts Valley Authority Sequoych Nuclear Plcnt LER SUPPLEMENTAL INFORMATION SQRO-50-327/ 8033 Technical Specification Involved 3.1.2.3 Reported Under Technical Specification 6.9.1.13.c Date of Occurrence: 4/11/80 Time of Occurrence: 0940 CST Unic 1 Identification and Description of Occurrence:

CCP l-A was removed from service and CCP l-B placed in service with D/G l-B out of service. With D/G l-B out of service, CCP l-B was being powered, by definition, from an inoperable shutdown board, even though its normal and alternate supplies were operable.

Conditions Prior to Occurrence:

Unit 1 in Mode 6 with all fuel loaded and reactor head on and no Core Alterations being performed.

Apparent Cause of Occurrence:

Personnel did not verify D/G operable on the shutdown board from which the CCP was being powered during Mode 6.

Analysis of Occurrence:

There was no hazard to the public health and safety.

Corrective Action:

Personnel was instructed as to the requirements of LCO 3.1.2.3.
