Dear Sir or Madam :

In accordance with 10 CFR 70.74(b), Global Nuclear Fuel-Americas, LLC (GNF-A) hereby submits a written follow-up report for Event Notification 55216 which was provided to NRC on 4/27/2021 (Reference 2).

Event 55216 As discussed in the initial event report, GNF-A reported a fire barrier impairment to the New Hanover County (NHC) Deputy Fire Marshall. Consistent with 10 CFR 70, Appendix A(c), a concurrent report was provided to NRC and is included as Attachment 1 to this letter.

Additional information is provided as follows:

Event Details and Safety Significance At approximately 1822 EDT on April 26th, the New Hanover County Deputy Fire Marshal was notified that the Fuel Manufacturing Operation (FMO) building fire doors on the first and third floor of the Dry Conversion Process (DCP) elevator shaft malfunctioned and were left in the open position to allow for the elevator repair contractor to observe the issue. The DCP elevator is located on the South wall of DCP which is a credited fire barrier. The condition did not present a significant safety hazard. Because the New Hanover County Deputy Fire Marshall was notified, a concurrent notification to the NRC Operations Center was made per 10 CFR 70, Appendix A( c}.

US NRC M210083 June 24, 2021 Page2 Immediate Corrective Actions Taken A fire watch was initiated and maintained until the elevator doors were restored to service at approximately 1330 EDT on April 27th.

Probable Cause of Event The electronic controls of the elevator door were intermittently malfunctioning, and the doors were left in the malfunctioned position to facilitate troubleshooting.

Short Term Corrective Action

1) The system was restored to service after the elevator repair contractor was able to observe and repair the issue. The New Hanover County Fire Marshall was notified at 141 0 EDT on April 27th that the doors were returned to operational status and that the fire watch had been terminated.

Longer Term Corrective Action

1) Update relevant procedure to include more detailed instructions for internal notifications of fire impairments and fire watch requirements.

2) Review Fire System Impairment procedure with appropriate personnel.
