Morning meeting brie£ Supervisor (V.Baldwin) asked if two FSS technicians could stay over tonight to perform a gamma walk over survey. The survey will be to verify contamination before a Radioactive Materials Area (RMA) can be down posted per procedure ZS-RP-106-000-000. The radioactive materials area is m survey unit 12201. The FSS technicians that will be performing the survey are Karen Nicholson (KN) and Chandler Williams. (CW)
MARSAME manager (C.Fuller), and FSS technician (KN) met with Op's RP Manager (D. Nichols) to discuss the job scope for this evenings work.
The scope: Waste Op's and RP Op's are removmg the radioactive material waste from an existing RMA, and the RP Op's will perform their survey inside the posted RP boundary. When the RP Op's complete their survey, the posted RMA boundary will remain up until the FSS technicians perform a Gamma walk over survey per procedure. It had been discussed we would get on our general RP R WP to perform this survey since it will still be posted.
Preparing equipment needed for upcoming survey, and pre source checked, Model 2350-1 #304708, cal due date 4-1-16, with a 44-10 probe #PR321892.
Passed source check.
FSS technicians went out to work area to meet up with Op's RP supervisor (P. Johnson). When t.1ie FSS technicians arrived in the field there were no boundaries in place to show where the RMA boundary was for survey. The Op' s RP supervisor had Op' s RP place stanchions that were still in the area, out to show were the boundary would have been. There was one radioactive box still in the area that was posted contact* 17/8@ 30cm mr/hr that had an impact on our backgrounds (15-25k cpm). The Waste Op's foreman and Op's RP supervisor were able to relocate that box to the fenced in Radiation Area in the south array.
There was work still being performed at the containment doors, but would be finishin$ soon. The FSS technicians signed in on their RWP and performed a two mmute drill in work area.
Collected backgrounds for survey: 2163, 2269, 4073, 2919 cpm, average 2856 cpm. MDCR 571 cpm, ASP 3427cpm, and the scan MDC 28,563 dpm/100cm2.
Originally the collection of the background was to perform the gamma walk over survey, but just approaching the southwest corner to perform background, a particle was identified (outside the stanchion placement). The particle was 300k cpm in the gravel, and was collected. The stanchions for the scan area were moved out to meet up where the crane boundary was.

It was apparent that there were multiple particles in the area and a gamma walk over scan for release could not be performed. The use of the alarm set point was disabled. The scan survey is scanning, and identifying the particles, and removing them. There were a total of 16 particles found and only just around 40% of the survey area had been completed on the north end outside Unit 2 containment. We were losing daylight. We collected 3 of the 16 particles, (attempted to collect particle #10, but was embedded in the concrete, no success), ioentified the rest with markings. There was an area on and between the cement slabs that was distributed 6k cpm to 12k cpm scan using the 2350-1/44-10, but a smear on it was <1000 dpm/I00cm using the Model 3 #283280.
Called the MARSAME manager (C.Fuller) to update him on the situation, change in job scope, and what should be our further action. C. Fuller said "He is coming back into work that night to help with the situation". Once he arrived and saw the situation, he called the Director of Radiation Protection (C.Keene) to also inform him on our findings.
The night shift Op's RP Supervisor (G. Parkhurst) had Op's RP technicians repost the area. The FSS technicians continued surveying, but when it got dark, left the marking on the particles identified and left the area. The 3 particles captured were put into the RCA for disposal, and the FSS technicians went through PCM, and signed off the RWP. Model 2350-1 passed post source check.
MARSAME manager (C. Fuller) called and asked ihe FSS technicians to sign out a Model 2350-1, with the 44-10 probe to the Op's RP, so they can use it for informational purpose to continue capturing particle throughout the night. FSS technicians brought the meter to RP Op's and turned over our information to them where we left off. In the turnover we gave our draft map showing locations of the particles identified by FSS technicians, and Op's RP would be continuing to remove the particles thru out the night.

Morning meeting brief. The PSS group will stop performing structural MARSAME surveys, and perform gamma scans for the entire 12201 survey unit. There is a contract company coming in to dump truck loads of gravel on the same area that was posted Radioactive Material Area (RMA) the night before.
The gravel is needed because the crane is being moved slightly east of where it laydown now, and the wavel with a wood plank platform will be made. The RMA postmgs are down, and per PSS/LT manager the gravel will be allowed to be dumped, but the rest of the area will have the gamma scan for particles performed.
All PSS technicians are scanning in the field starting at the Aux building roll up door. This area is survey unitl 2104, and particle #2 (400k cpm) was identified and removed.
The rest of the particles were found in survey unit 12201.
There were a total of 31 particles found and removed. The 2 highest particles are bolded. Here is the list of 30 particles found in surve unit 12201 and 1 from 12104:

The Unit 2 turbine truck bay and the walkways from SU12201 to 10203 PM-7' s were surveyed and no particles found. All particles were disposed in the RCA radwaste.
There was a green hopper inside of an RMA (Area 1) that was relocated, across the road into a new RMA (Area 2), in SU1220 I. A survey will be performed by the PSS technicians, Jeff Graham and Chandler Williams, before the RMA boundaries can be down posted per procedure ZS-RP-106-000-000. The PSS survey identified contamination soil distributed in a low laying area, and two particles. The particles were identified as #16 and #17. The distributed soil scan results show lOk-14k cpm after particles were removed. The PSS technicians collected four soil samples for gamma spectroscopy. This soil has not been removed awaiting, gamma spectroscopy reports attached. Survey #2015-M-0-182. New gravel was placed on top of the contaminated soil as per MARSAME manager instructions.

Antonio evaluated the backgrounds in the fuel handling car shed. South back wall at the grey doors is 84k cpm, and the north roll up door opening drops to 20k cpm.
There are two RMA boxes outside the car shed, the south one has a background of 3.5k cpm, and the north one is 40k to 95k cpm.
Adam Huenink identified 35k cpm distributed elevated levels that run along a crack in the asphalt that could not be remediated at this time. The area was marked with pink flag/whisker. This area is at the edge of the road on the northeast side of the survey unit, in front of Pullman's break area building.

Morning meeting brief with supervisor P. Giza. There are limited work crews in the working today because this is not a normal scheduled work day. The Pullman group and Manafort are working today (a lot of vehicle traffic though).
The scope of today's work is to scan as much as we can for Survey Units (SU) 12202, and adjacent units 12201, 12107, and 12108. When we identify a particle, we will capture it if possible, denote level, location, and/or ifit can't be retrieved. The average background range 2.5-5k/cpm.
Started survey where we left off Thursday ( 6-4-15), and cal'turing a few areas from 12201. There are four FSS technicians performmg sun.-eys today. Ted Schlueter scanned the far northeast strip of survey unit 12201.
There were two particles identified in the gravel along the fence in this area. The particles were identified as #1 (10k cpm) and #2 (56k cpm),
and both were removed.
Jim Green surveyed and collected particles in surYey unit 12201. There also is an area that has distributed levels with 8-12k cpm that we marked with pink whiskers until further action decisions are made. This is the same area identified on 6-3-15. There were two particles identified as #3 (40k cpm) and #4 (60k cpm), and removed. Jim and Antonio continued to survey the road areas in survey unit 12202. There were nine particles identified in this survey unit and removed. They are identified by #5 (7k cpm), #6 (70k cpm), #iO (80k cpm), #12 (108k cpm), #13 (140k cpm),
- 14 (72k cpm), #15 (240k cpm), #f6 (50k cpm), and #18 (90k cpm).
Technicians Karen Nicholson and Antonio Torres met with supervisor for Pullman to discuss scanning around them in their work area. He had given us permission, but met up with their foreman Jose for job specifics.
We were able to blend our scannins needs in with their work. The foreman gave me permission at thetr lunch to go behind their boundary and scan. Pullman work area is in survey units 12107 ( closest to the fuel handling building south exterior) had no particles identified. Pullman's staging area and temporary restrooms are in survey unit 12108, and there were five particles found in this survey unit. Three of the particles identified as #8 (200k cpm), #9 (350k cpm), and #11 (200k cpm). The other two were embedded more in the concrete pores, and are identified as
- 7 (30k cpm) and #17 (120k cpm). This area has been used for a laydown ofradioactive material for most of two years. The area was posted as a RA and RMA in the past.
While a technician was scanning the road, NRC walked by and mentioned the scan speed could be a little slower. A contributing factor was there were strong winds that affected the probe stability.

The northwest strip of survey unit is complete. The technicians continued south by evaluating backgrounds from a radioactive material area filled with RM containers, and the two RM liners stored in the area. When we started to evaluate the area, workers in unit 1 containment moved a box out on to the Bigge rail and staged it there. This caused our backgrounds to more than double. We moved on to the south vehicle gate thru way. The backgrounds were 7k to 1 lk in the area, but completed the scan survey in there. No particles were found.
We had four Model 2350-1 pre and post source checked for survey today.
All four passed all source checks. Meters used:

Morning meeting brief.
Multiple survey units inside the security fence on the north, west, and south side of the site had gamma walk over scan surveys performed (using a Model 2350-1 with 44-10 probe) to identify and remove any particles that were found.
There was a total of 75 particle found from the surveys performed on 613, 614, 615, 618, and 619. A breakdown of each day with totals:

Keene has requested that any particle found equal or greater than SOOK cpm be saved and submitted to count room.

CR-2015-000324 Keene.Chris 6/4/2015 12:08 PM ISSUE TITLE:Describe the issue in a brief (about 10-12 words) problem statement.
Contamination Identified on the Ground on the North Side of Unit 2 CNMT ISSUE DESCRIPTION:Briefly describe the condition and provide background information as appropriate (optional).
A temporary radioactive material area was created on the North side of Unit 2 CNMT to contain several segmented pieces of Unit 2 Steam Generator secondary side components. This material was loaded into rail cars for shipment on 06/02/2015 and 06/03/2015. Once completed, the area was surveyed by RP Ops personnel. No contamination was identified and the area was downposted. This downposting occurred around 1600 on 06/03/2015. This was in violation of procedure ZS-RP-106-000-000 "Radioactive Material Control Program" procedure as FSS personnel are required to evaluate the area prior to deposting. FSS personnel were contacted previously, and, when arriving to survey, found the area already downposted. RP Operations personnel were contacted and the area was reposted.
The area was not posted for approximately 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br />. During surveys by FSS, several areas of contamination were identified. RP Management personnel were immediately contacted who then contacted the Control Room. D&D work was stopped in both CNMTs and all RPTs were redirected outside to the area to resurvey. Some additional locations were identified. 24 locations were identified in all, highest was 500k dpm, lowest was 20k dpm and average was around 50k dpm. All areas were remediated. After obtaining concurrence with FSS/L TP Manager in the morning on 06/04/2015, the area was then downposted. The survey instruments RP Operations uses are gm tube detectors which primarily identify beta emissions from contamination. FSS uses sodium iodide detectors which identify gamma emissions from contamination. The contamination identified and remediated was near surface to several inches below surface. This results in most or all beta emissions being shielded and not able to be located v.ith gm detectors as the primary radionuclide present in the contamination is Co-60. This is the primary reason no contamination was identified by RP Ops during earlier surveys. Nal detectors were loaned to RP Ops to perform subsequent surveys for identification and remediation of contamination in the evening of 06/03/2015.
What immediate corrective actions were taken? [ ]N/A Contacted Control Room to screen for reportability. The highest contamination identified was 500k dpm. For reportability screening, a bounding assumption of 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of skin contact was assumed which could have resulted in worst case 2 rem of skin dose. That level would not have violated any site administrative or regulatory limits. That level of SDE would not be deemed feasible due to contamination being subsurface, the time the area was down-posted was approximately 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> and all personnel exiting site pass through the PM-7s which would have identified the contamination. Therefore, the 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> was for bounding purposes only. Briefed all RPSs and RPTs on FSS evaluation requirements for downposting RMAs on 06/04/2015 during weekly safety brief.
What further corrective actions do you recommend? [ ]N/A The contamination levels on the S/G secondary side pieces previously located in the area were less than 1 Ok dpm/100cm2 removeable and fixed. The contamination identifed on the ground is not believed to have originated from that RAM. Therefore, the improper temporary down-posting of the area cannot be directly tied the consequences of identifying the radioactive material that existed prior to inttially posting the area. The contamination was primarily Co-60 in nature and would have originated from primary side components in Unit 2 CNMT. The date and exact cause for spread of contamination cannot be detemiined. This area and the similar area on the south side of Unit 1 CNMT are known to have low levels of contamination from earlier FSS surveys. Recommended action is to have FSS perform Nal walk over surveys of laydown areas outside Unit 1 and Unit 2 CNMTs, as radiological background conditions allow, to identify and remediate any discrete radioactive particles and contamination identified at levels based upon concurrence of Director of Radiation Protection and FSS/L TP Manager.
