SURVEILLANCE COMMENTS AND OBSERVATIONS: This survey unit was accessible after the removal of the CCDD pile. The FSS technician contacted the MBI foreman (N. Friant) for support, entered the survey unit to perform the walk down and select the sample locations. The MBI laborer support will join the FSS technician when they become available_ 14:55 The FSS technician received MBI laborer support (Tim C) and setup at the first biased surface soil sample location. 15:00 Collected biased surface soil sample #Ll 12112AVJGS001SS.

== Description:== light/medium brown sand, and gravel. Location was taken on the footprint of a small piece of metal that was removed from survey unit. The metal was surveyed, result, indistinguishable from background. 15:10 Collected biased surface soil sample #Lll2112AVJGS002SS.

== Description:== light/medium brown sand, and gravel. Location selected at tire marks and water mark area. 15:25 Completed surveillance survey. The group signed out of the survey unit. The collected samples were brought to the sample prep trailer for processing. Notes: The sample locations were GPS'd, and the corrected coordinate with the location will be shown on the attached map. There were some small pieces of plastic and metal, water bottles, and some old flags surveyed and removed from the survey unit. The postings were satisfactory. The FSS technician will perform the planned surveillance survey (walk down and sample), but will also include a gamma walk over survey on the traverse path. 13:30 The FSS technician and the MBI laborer support (RD, TO) contacted the MBI foreman (N. Friant), and signed into the survey unit. Friant), signed into and surveyed into the survey unit This survey is the 1st Bi-annual surveillance and a special survey on a pile of dirt staged at the FSS boundary (See attach sheet for special survey instructions for the dirt ile. 08:30 Performed the scan survey on the pile and IO meters square on the perimeter at the track marks. Backgrounds; 3392, 3827, 3559, 3306, and 3481 cpm, average 3513 cpm, MDCR 634 cpm, and ASP 4147 cpm. There were no alarms for all scan results (3" collimator used). SURVEILLANCE COMMENTS AND OBSERVATIONS: This survey unit was part of the traverse path to remove the CCDD pile. The CCDD is now removed, and the surveillance survey is being performed. The FSS technician will perform the planned surveillance survey (walk down and sample), but will also include a gamma walk over survey on the traverse path. The laborer support will be available after lunch, so the FSS technician will be going in and perform the walk down and complete the gamma walk over survey. Informed the MBI foreman (N.Friant). 08: 15 The FSS technician signed in and surveyed all the sampling equipment into the survey unit. The FSS technician established the scan area and started with verifying the backgrounds. The west side has a higher background due the media having more clay surface, so there will be two set of backgrounds making two scan areas, #1 and #2. Scan Area #I backgrounds: 2103, 2117, 2208, 2284, and 2116 cpm, AVG 2166 cpm, MDCR 497 cpm, and ASP 2663 cpm. Scan Area #2backgrounds: 3003, 3526, 3590, 3572, and 3413 cpm, AVG 3421 cpm, MDCR 625 cpm, and ASP 4046 cpm. Completed the gamma walk over scan, lunch. 12:30 MBI laborer support (RD, AV) joined the FSS technician to complete this survey. == Description:== Performing 1st Bi-Annual surveillance survey. Survey Unit Type: Structure D Classification: OpenLand!ZI Class 3 D Class2 D Class 1 ~ Reason for Surveillance: Semi-Annual IZI Special D System Surveillance Due Date: 7-10-19 Reason: Bi-annual Surveillance survey. (Survey Date surveillance performed: 8-1-19 performed after due date because the storage and removal of the CCDD pile in the adjacent survey area 12205). SURVEILLANCE ACTIVITIES:

1) Review ZS-LT-300-001-003 Attachment 2, FSS Area Access Request, Authorization and Sign-In Sheet for authorized entries into the FSS Survey Unit*--*-----------------------------------------------------------------------------------------* IZ!
2) Perform a walk down of the FSS Survey Unit and identify decommissioning activities adjacent survey units that could have a reasonable potential to spread radioactive material to the FSS Survey Unit--------*------- IZ!

SURVEILLANCE COMMENTS AND OBSERVATIONS: The CCDD is now removed, and the surveillance survey is being performed. The FSS technician and the MBI laborer (RD, AV) support walked down the survey unit and selected sample location. Informed the MBI foreman (N.Friant). 13:38 The FSS technician signed the group in, and surveyed all the sampling equipment into the survey unit. 13:52 Collected biased surface soil sample #Ll 12203CVJGS001SS.

== Description:== medium brown sand, and gravel. Location selected at a dried, low water area. 14:15 Completed surveillance in this survey unit. The group signed out of the survey unit. The collected sample was brought to the sample prep trailer for processing. Notes: The sample location was GPS'd, and the corrected coordinate with the location will be shown on the attached map. There were some small pieces of metal, and old flags that were surveyed and removed from the survey unit. The postings were satisfactory. The postings are represented on the adjacent FSS survey units. A Model 3 #283228 with a 44-9/PR355606, cal due 1-12-20 was used for all the surveying.
