Results of Re-Survey of Power Block Area and Readiness for ORISE Confirmatory Surveys The soil around the footprints of the Unit 1 and Unit 2 Containments is what is referred to as the Power Block area. The Power Block consists of 19 Class 1 open land survey units for Final Status Survey (FSS). These survey units are as follows; the southern half of 10203A, 10203F, 12201A, B and C, 12202A, B, C, D, E and F, 12104, 12105, 12106, 12107, 12108, 12109, 12110 and 12111. The approximate current grade of the Power Block indigenous soils is the 588 foot elevation.

On April 15, 2019, the NRC and ORISE mobilized to Zion to perform a Confirmatory Survey of the Power Block area. The purpose of the Confirmatory Survey was to ensure that ZSRP had successfully remediated the Power Block area to the Operational DCGLs for subsurface soils after removal of sacrificial soil and prior to the planned increase of grade to the 591-foot elevation (which is the LTP established compliance grade elevation for buried basement footprints). Primarily due to a work order issue for control of the D&D remediation work scope, the NRC and ORISE were not informed that the areas over the Spent Fuel Pool, Car Shed, Truck bay and the Auxiliary Building basement were not part of the sacrificial soil removal scope and subsequently, were not scanned. These soil areas which were not remediated or surveyed were located in survey units 12105, 12106 and 12107. An extensive Issue Review was conducted for Condition Report ES-Zion-CR-2019-0046 to identify corrective actions to prevent reoccurrence.

On April 16, 2019, the project was notified that ORISE had discovered a discrete particle (536K cpm, 1.74 uCi) in open land survey unit 12106. Another discrete particle (80K cpm) was discovered in survey unit 12106 by C/LT Technicians the following day during the performance of surveys to support the discovery of the 1st particle. These particles were located within the soil areas that were not previously remediated or surveyed.

Upon discovery of the second particle, ZSRP Project Management requested that the in-progress Confirmatory Survey by ORISE be suspended. Consequently, ORISE and the NRC exited the site on Thursday, April 18, 2019. As a result of the discovered particles, the miscommunication over what was part of the survey scope, and the requested suspension of the Confirmatory Survey, a Condition Report (CR) was generated. CR-2019-0046 entitled, Discrete Radioactive Particle found in Open Land Survey Unit 12106 was issued and an Issue Review (IR) was performed as mentioned above.

In response to the IR, planned action #3 states, Perform a 100% scan of the exposed soil surface in the 19 open land survey units that make up the Power Block. The area will be re-scanned using 44-10 detectors in accordance with an addendum to Radiological Assessment (RA) 12000-Page 1 of 4

F-01, which will require the survey of all exposed soil surfaces, not just the surfaces exposed by the sacrificial soil removal. All scan measurements that exceed the alarm set-point (MDCR plus background) will be flagged and sampled for isotopic analysis. All soils identified as exceeding the Operational DCGLs for subsurface soils from LTP Chapter 5, Table 5-8 will be remediated.

A final scan will occur, along with soil samples as indicated by scan to ensure remediation is complete and successful. In addition, planned action #5 requires the Zion General Manager to conduct and document a readiness review to verify that all planned action in preparation for ORISE have been satisfactorily completed. Completion of the FSS aspects of planned action #5 is documented by this summary paper and the attached survey documentation.

The resurvey of the Power Block soil surfaces commenced on Thursday, April 18, 2019 under Sample Plan 12000-F-01 in accordance with procedure ZS-LT-200-001-001, Radiological Assessments and Remedial Action Support Surveys. Technicians using Ludlum Model 44-10 NaI detectors scanned the soil surface at a scan speed of 0.25 m/s within 1 inch of the surface.

The alarm set-point was set at the Minimum Detectable Count Rate (MDCR) of the instrument plus the background. In accordance with the sample plan instructions, a minimum of 5 background measurements were obtained in each area prior to commencing scans and the average was used to establish the alarm set-point.

Due to the fact that the subsurface soil DCGLs are very low, any increased background greater than approximately 3000 cpm will hinder the Scan MDC of the 44-10 to approximately 90% of the subsurface soil Operational DCGL. Due to the presence of large volumes of clay which increases background radiation due to Naturally Occurring Radioactive Material (NORM), the 44-10s were equipped with collimators to attempt to reduce the background contribution.

However, in most of the survey units within the Power Block, the background was greater than 3000 cpm. Consequently, it is possible that if residual radioactivity was present in the soil being scanned at a concentration at, or slightly above the subsurface soil Operational DCGLs, that the scans would not detect it. In order to compensate, due diligence soil samples were taken in each survey unit at the locations exhibiting the highest scan readings. The soil samples were then analyzed by the on-site gamma spectroscopy system. In addition, on May 3, 2019, Sample Plan 12000-A-09 was written to adjust scan speeds, MDCR and alarm set-points for soil areas exhibiting high concentrations of clay fill during subsequent remediation efforts.

From Thursday, April 18, 2019 through Tuesday, May 28, 2019, the C/LT Group comprised of C/LT Technicians and Supervisors performed a RA of the accessible soil surfaces within the Power Block survey units. Due to the presence of large amounts of clay, the background in most of the 19 survey units that comprise the Power Block was elevated to levels exceeding 3,000 cm.

Consequently, a surface soil sample was acquired in each contiguous scan area at the location exhibiting the highest scan reading.

Several areas were identified by scan and verified by media sample analysis that required remediation. These areas were focused primarily on the soil footprint where the Containment Tents were constructed and from the areas where radioactive waste was stockpiled prior to Page 2 of 4

packaging. Large excavations were created to the south of unit 1 Containment and to the north of Unit 2 Containment. Identified radioactive soils with concentrations up to 50% of the Operational DCGLs for subsurface soils were marked and removed for proper shipment as radioactive waste. Post remediation scans and media samples established that the remediation was successful and the level of residual radioactivity remaining in the soils was less than the Operational DCGLs for subsurface soils.

Additionally, low level particles were seen at shallow depths in similar areas adjacent to the containments and former tent areas. These areas were treated conservatively in terms of large bounding remediation footprints, and soil samples to bound and confirm that there were no plant derived radionuclides present in or adjacent to these clay excavation areas above the Operational DCGLs for subsurface soils. Scans also revealed a piece of concrete about the size of a softball in survey unit 12104 that read 10K cpm. All particles were recovered and additional scans and investigative soil samples were taken to ensure remediation was successful.

Upon completion of the remediation of all identified areas where elevated activity was detected, 100% of the exposed soil surface was completely scanned to verify that the residual activity in soils was less than the MDCR of the instrument plus background. In addition to the remediation support soil samples described above, due diligence soil samples were also taken in all areas where elevated scan measurements occurred, primarily areas with significant clay present. Any areas where scans showed greater than MDCR plus background received a soil sample, and clay removal/remediation and bounding soil samples as needed. The result of the final set of media samples representing the post remediation and current as-found condition of these survey units were all less that the Operational DCGLs for subsurface soils.

On Monday, May 20, 2019, a readiness walk-down was performed of the Power Block.

Participants included the D&D Manager, D&D Projects Manager, Lead C/LT Engineer, C/LT Supervisor and the Site Closure Director for EnergySolutions. As part of the walk-down, any areas with large amounts of clay, any large pieces of concrete or demo debris lying on the surface or any other visual indications of potential issue were noted. During the walk-down, a concern was brought by the Site Closure Director that the numerous excavators and bulldozers that had traversed the area during the remediation effort could have potentially exposed additional particles. Consequently, additional scans were performed over any soil surface that was disturbed by a tracked vehicle. The scans were completed and no alarms were detected. In addition, another comment was made that the ropes used for I&C were degraded and needed replacement. The last comment pertained to a section of soil in survey unit 12111 that was marked as elevated but was not remediated. Additional scans were performed of the identified area. During the scans, excessive background was again experienced over soils that contained a large amount of clay. To compensate, additional soil samples were acquired at the locations exhibiting the highest scan results. All vehicle traffic was suspended inside of the Power Block footprint and will remain suspended until the ORISE confirmatory survey is complete.

From Thursday May 23 through Tuesday, May 28th, the remaining investigation and post remediation soil samples were analyzed by the on-site gamma spectroscopy lab. Plant derived radionuclides, while still detectable in some areas are less than 50% of the Operational DCGLs for subsurface soils. See attached spreadsheet detailing results of relevant soil sample analysis supporting the power block remediation.

Based on the results of the scan surveys, sample analysis and visual observations that are summarized above, 100% of the surface soil in the Power Block area was subjected to scanning using a 44-10 NaI detector and media sampling. The areas scanned and the observed average background for different areas is denoted on Figure 1. Investigation and due diligence samples of the soil surface taken at locations biased toward the discovery of discrete particle or, elevated scan measurements indicated the presence of elevated soil activity and in some cases, at concentrations that were greater than the Operational DCGLs for subsurface soils. Remediation was performed to recover discovered discrete particles in survey units 12104, 12106, 12107, 12109, 12201B, 12201C, and 12202D or, to reduce detected volumetric contamination in soil in survey units 12104, 12105, 12106, 12107, 12108, 12109, 12110, 12111, 12201A, 12201B, 12201C, 12202C, 12202E, 12202F. Post remediation samples indicated that Cs-137 was present, however at concentrations in soil commensurate with what can be considered background in accordance with TSD 13-004, Examination of Cs-137 Global Fallout in Soils at Zion Station.

No vehicle traffic will be allowed in the Power Block Survey Units until after completion of ORISE confirmatory survey work.

All survey data generated under RA 12000-A-09 was reviewed and approved by C/LT Supervision in accordance with procedure ZS-LT-200-001-001, Radiological Assessments and Remedial Action Support Surveys. In addition, all survey documentation and field logs were reviewed by the C/LT Manager in preparation for this readiness review. Maps of scan results were reviewed and confirmed to be complete over 100% of the exposed areal soil surface within the Power Block.

Based on the results of the surveys provided and an analysis of the results, it can be concluded that the exposed soil surface within the 19 open land survey units that comprise the Power Block is ready for the recommencement of the Confirmatory Surveys of the Power Block soils by ORISE.
