k This investigation revealed that the allegation of harassment and intimidation of an L.K. Comstock QC inspector was not substantiated.
Information was developed to show that once the alleger better understood the L.K. Comstock i
record turnover procedure, and the significance of a QC inspector signature on the turnover documents, the concerns of this alleger of being forced to sign these documents were alleviated.
As to the second allegation, however, which alleged that the signature of a QC inspector was forged on three turnover documents, information was developed to substantiate that an L.K. Comstock QC supervisor did in fact forge the signature of a QC inspector on three initially identified turnover documents.
Additionally, in spite of the QC supervisor's under oath affirmation that only the three documents initially found were instances where he forged a QC inspector's signature, further 01 investigation revealed a fourth unreported forgery committed by the QC supervisor.
This investigation has been closed by 01.
This report has been forwarded to you for your information and whatever action you deem appropriate. You should also be aware that a copy of this investigation has been forwarded to the Department of Justice for a possible criminal prosecution. Neither this memorandum nor report contents may be released outside the NRC without the permission of the Dir&ctor. 01.
