No documentation to verify that ultrasonic machines are in horizontal or vertical linearity. RE: AWS D1.1 1972 Idition 2. Flute heads in Airlock and Steam Tunnel - About 24 welds that l require UT inspection - Inspection could not be performed as documented. At least 22 or the 24 have incomplete fusion. h 3. NES was asked to perform X-ray inspection of jet pumps. Due to configuration of equipment, an " oversize" film was utilized. { _ The welds that were the targets of the inspection were satis-i factory. but a number of wel adjacent to e target welds j were found to be rejectable. (Wismer & Becker) requested NES to cut film to iminate rej t welds. The film and reports have been manipulated to conceal rejectable welds. A survei ance report va issued on this but the number is unknown. above situation. of PQA was also involved in the j ~ 4. A number of UT reports purportedly signed by the interviewoo are forgerins. The licensee it. currently investigating these allegations. RE: AWS D1.1 1972 Edition." 4 ) During the investiEation of your concern it was found that no evidence existed confirming that horizontal linearity checks were performed af ter each 40 hours of instrument use. Also, there was no information available to confirm that ultrasonic test instruments gain control g settings were being checked at two uonth intervals. As a result of this finding two Deviation Disposition Reports were initiated ((W) 12611, (W) 12612) by Project Quality Assurance. These l were initiated for Edison Engineering to evaluate whether these dis-C crepancies have any effect on the structural steel welds installed by Walbridge-Aldringer Co. that were previously examined by ultrasonics and to conduct training with responsible personnel to establish con-trols to prevent repetition of this occurrence. When complete, documentation will be available from Fermi 2 wite Record Vault. B) The calculated.002 inches of excess geometric unsharpness is negligible and does not interfere with the ability to determine final acceptance of the radiographs. The geometric unsharpness equation is included in the ASE Code for information and guidance, to be applied within practical limits. Accept a s-is. C) The condition of alig1ed or oversized porot,ity does not exist in at:y of the Jet t l Pump welds. That is, rounded indications that touch a line parallel to the length of the weld drawn throug1 the center of the two outer rounded t indications or maximum pore sizes that exceed the ASE Code. Proposed Resolution l A) Internal root weld conditions are acceptable when the density changes as t indicated on the radiograph are not abr@t. Accept as-is. B) The calculated.002 inches of excess geometric unshamness is negligible and does not interfere with the ability to determine final acceptance of the radiographs. The geometric unsharpness equation is included in the AS{ Code for information and guidance, to be applied within practical limits. Accept a s-is. C) The condition of alig1ed or oversized poros,ity does not exist in any of the Jet Pump welds. That is, rounded indications that touch a line parallel to the l 1ength of the weld drawn througi the center of the two outer rounded Indications or maxirwm pore sizes that exceed the ASbE Code. Accept as-is. l I D) Film side placement of the penetrameter was determined by the radiographer to be the most practical method due to weld configuration and radiographic te::hnique utilized. The penetrameter image and specified hole is oiscernit,le on the radiographic film. Accept as-is. 3 E) All original radiographic indications associated with Jet Pump weld joint numbers 1 through 20 were interpreted by a GE NDE Level III and found to be within the acceptable limits of the ASPE Code. A recent additional evaluation of Jet Pump weld joint number 18 by GE has been interpreted as incomplete fusion. A-fracture evaluation'of the jet ptrp diffuser to adapter weld has been i performed by General Electric Design Engineering to determine the maximum allowable circumferential surface crack on the O.D. of the weld. It has been determined that crack propagation will not occur under the following conditions: 1-For ncuninal wall thickness of.375", a maximum crack size of.225" provided the crack depth is less than.037". 2-For minimum wall thickness of.340", a maximurn crack size of.204" provided the crack depth is less than.034". We have requested, as part of the Final Disposition on PDDR KH1-258, that a P.T. exam be made on the backside of these welds. 'Ihis is Detroit Edison's final report on the defective GE-ISSE welds problem. The problem was originally reported to Mr. P.A. Barrett of NRC Region III by Detroit Ediscn's Mr. T.A. Alessi, Director-Project Quality Assurance on October 24, 1980. At that time, a review of nineteen (19) radiographs in the G-33 system, which is a drain line to the reactor vessel, revealed problems with seven (7) out of the nineteen (19) welds. These radiographs were previously ac pted by a contractor that had since left the site. i. Following an investigation, it was determined that additional defects had been noted in reviewing radiographs of other previously accepted GE-IASE welds. The welds were jet pump diffuser to adapter welds and safe end rnodification welds. Repair work on the defective welds has been ecznpleted. The welds are now acceptable.
