INTRODUCTION During the scheduled Sequoyah Nuclear Piant (SQN) Unit 1 Cycle 8 refueling outage (U1C8 RFO) extensive inservice inspections were conducted in all four steam generators (SGs) to address all active and potential damage mechanisms identified in the U1C8 Pre-outage Damage Assessment. The results of the inspections were classified as C-2 for all four SGs. Alternate plugging criteria (APC) implementation continued during this inspection due to the detection of outside diameter stress corrosion cracking (ODSCC) at tube support plate (TSP) intersections.

A total of 15 tubes were in-situ pressure tested,5 in SG 3 and 10 in SG 4, to -

support structural and leakage integrity evaluation for U-bend primary water stress corrosion cracking (PWSCC), TSP PWSCC and ODSCC, and top of tubesheet (TTS) PWSCC.

This report fulfills the repor'ing requirements of SON Technical Specification Section 4.4.5.5.b for recorting results of SG inservice inspection.

This report fulfills the reporting requirements of SON Technical Specification.

License Condition 2.C(9)(d) to provide results, distributions, and evaluations -

SG TUBE INSERVICE INSPECTION SCOPE AND RESULTS The SON SG tube Inservice inspection (ISI) initial sample and expansion for all SGs and all damage mechanisms were as follows:

100% full-length bobbin examination in all 4 SGs for initial screening and detection of indications 100% hot leg TTS (WEXTEX transition region) examination'in all 4 SGs with 0.080 mid range /0.080 high frequency / 0.115 mid range rotating pancake coil (RPC) probe.

100% Row 1 U-Bend and 20% row 2 U-Bend examinations in all 4 SGs with a magnetic biased ZETEC standard Rcw 1&2 U-Bend RPC probe.

100% >=5 volt dented TSP intersections from H01 to H07 in SGs 1,2, and 4 with + Point probe.

100% >=5 volt dented TSP intersections from H01 to H03 and a 20%

random sample of H04 in SG 3 with + Point probe.

100% < 5 volt dented TSP intersections from H01 to H02 and a 20%

random sample of H03 in SGs 1 and 2 with + Point probe.

100% <5 volt dented TSP intersections from H01 to H03 and a 20%

random sample of H04 in SG 3 with + Point probe.

100% <5 volt dented TSP intersections from H01 to H04 end a 20%

random sample of H05 in SG 4 with + Point probe.

All test techniques were EPRI Appendix H Qualified examination techniques.

The inspection of small dents was performed to envelope the highest TSP with identified PWSCC indications since trie PWSCC degradation mechanism is strongly temperature dependent. A tabulation of hot leg dented intersections at SON Unit 1 foi:ows. The shaded areas were tested in the initial sample.

MOO %YM 4129 Total Exams = 7624 Due to the detection of TSP PWSCC, the + Point examination scope in <5 volt dented TSPs in SG 3 was expanded to 100% of H04 and a 20% random sample of H05.

Table 1 summarizes the SON U1C8 eddy current testing ISI exams and the results of exams conducted. Table 2 provides a summary of the tube damage detected and a characterization of the damage morphology, Degradation Mechanisms Detected PWSCC U-Bend PWSCC in the Row 1 U-Bend area was not predicted in the preoutage assessment due to the low occurrence in Unit 1 SGs since U-Bend heat j treatment. Increased occurrence is due to the utilization of the magnetic bias U-Bend probe, which eliminated permeability variation masking.

Circumf;rcntini PWSCC was d:t:ct:d in the U-bend cres of 2 tubes. Th]

, longest and bounding indication, R1 C87 in SG 3, had a crack arc length of 117", which was below the 238 Regulatory Guide 1.121 structural limit.

Axial PWSCC was detected in the U-Bend area of 3 tubes. Two of the tubes exceeded the 0.47" length criteria and were in-situ pressure tested.

An axial indication at H07+10.26 in R1 C36 SG 3 had a crack length of 0.75" with a maximum depth of 65% through wall, average depth of 51%. Full-tube in-situ pressure testing was performed on this tube at 1600 psi (normal operating pressure),2800 psi (main steam-line break pressure), and 4750 psi (3AP). No leakage was detected and this tube exhibited a burst presse:? above the requirements of Regulatory Guide 1.121. An axialindication at H07+11.24 in R1 C84 SG 4 had a crack length of 0.99" with a maximum depth of 96% through wall, average depth of 74.4%. Full-tube in-situ pressure testing was performed on this tube. At 1600 psi, no leak was detected, at 2800 psi,113.9 gpd leakage was detected, and at 4750 psi,621.4 gpd was detected. This tube exhibited a burst pressure above the requirements of Regulatory Guide 1.121 and leakage below the 3.7 gpm accident leakage limit.

Condition Monitorina for U-Bend PWSCC The limiting U-Bend PWSCC indication for condition monitoring is R1 C84 (Figure 4). The predicted burst pressure for R1 C84 using a lower bound Row 1 flow stress for U-Bends heat treated in the field is about 4870 psi at operating conditions. The tube did not rupture at the 4750 psi pressure test. A SLB leak rate of < 0.1 gpm was measured for the room temperature test at 2800 psi. The only other potential contribution to SLB leakage results from the APC for ODSCC at TSP intersections for which a SLB leak rate of 0.28 gpm was predicted from the End of Cycle (EOC)-8 measured distribution of indications.

Consequently, the total SLB leak rate at EOC-8 is negligible compared to the allowable limit and the U-Bend PWSCC indications satisfy tube' integrity requirements for condition monitoring.

Operational Assessment for U-Bend PWSCC The U-Bend inspection at EOC-8 was more extensive in scope than the EOC-7 inspection, which did not include 100% of Row 1 U-Bends. Tube R1 C84 was not RPC inspected at EOC-7. The last inspection for tube R1 C84 was EOC-6, and review of data from this inspection indicates that an indication may have been present at this time. The R1 C84 indication, therefore, represents at least two cycles of growth. Given the 100% RPC inspection at EOC-8, the potential undetected indications left in service for Cycle 9 can be expected to be smaller than that for Cycle 8. Since operating conditions are essentially the same between Cycles 8 and 9 and all EOC-8 indications satisfied tube integrity requirements, it can be expected that all U-Bend indications found at EOC-9 will also satisfy Regulatory Guide 1.121 guidelines.

One tube (SG 3 R4 C51) displayed a distorted U-Bend indication on bobbin and was inspected with the 0.680" U Bend RPC probe. This indication was confirmed as axially oriented and displayed an OD phase angle and was subsequently plugged. The damage mechanism causing this indication is unclear. The most probable mechanism is Intergranular Attack (IGA) because the indication was more volumetric than crack-like.

PWSCC TTS A total of 100 tubes were predicted to be plugged for PWSCC at TTS based on SON and industry experience. Actual results show a decrease in PWSCC at TTS, which is attributed to shotpeening at the cycle 5 refueling oul age. The majority of these indications are in the central region of the '.ube bundle as predicted for WEXTEX cracking.

A total of 38 indications in 35 tubes were identified as circumferential PWSCC at TTS. The longest and bounding circumferential crack was 217 at HTS-0.12 in R24 C70 SG 3, which is below the 238 Regulatory Guide 1.121 structural limit.

One of the largest voltage circumferential cracks (3.98 volts-field data) was in SG 4 R12 C27. This tube was in-situ tested and exhibited a burst pressure above the requirements of Regulatory Guide 1.121. No leakage occurred.

A total of 21 in.;ications in 20 tubes were identified as axial PWSCC at TTS.

Three tubes exceeded the 0.47" length criteria; however, because the cracks were within the tubesheet where crack opening is limited by the tubesheet constraint, they were judged to be no challenge to Regulatory Guide 1.121 structural integrity or accident leakage.

ODSCC TTS Although SQN unit 1 has had low occurrence of ODSCC at TTS, based on industry experience, this mechanism was predicted to increase this cycle and 100% TTS exams were performed. A total of 30 pluggable tubes were projected.

Axial ODSCC was detected in only two tubes at TTS. Both were below the 0.47" length criteria for structural integrity.

Condition Monitorina and Operational Assessment for TTS PWSCC and ODSCC The PWSCC and ODSCC identified in the WEXTEX transition do not challenge buret or leakage integrity for EOC-8 or EOC-9.

1 Pvv3CC TSP Preoutage assessment predictions for PWSCC at dented TSPs were lower than actual results. Based on SON's experience,95 pluggable tubes were predicted. l A total of 41 circumferential PWSCC indications were detected in 3e tubes, and  ;

all were below the Regulatory Guide 1.121 structural limit. Adequate de;9ction of cirumferential indications at dented TSPs is supported by the prior tube pulls at SON Unit 1 and at Diablo Canyon Unit 1.

A total of 196 axial PWSCC indications were detected in 165 tubes. These tubes were screened for in-situ pressure testing, and 10 were determined to be the bounding cracks based on length, max depth, and voltage. These 10 tubes were in-situ pressure tested at 1600,2800, and 4750 psi. No leakage was detected and all 10 tubes exhibited a burst pressure above the requirements of Regulatory Guide 1.121.

The longest crack length indications outside the TSP were at R36 C42 SG 4 (0.31") and R14 C73 SG 3 (0.43"). The average depths for these indications are less than the 100% depth at 0.47", and adequate burst margins would be expected for these indications.

These indications were in-situ tested to 4750 psi and did not leak or burst, which confirms the conclusion based on NDE analysis. Therefore, all axial PWSCC indications at TSP intersections satisfy the Regulatory Guide 1.121 burst margin guidelines.

Burst cap:bility of th3 indications c:n clso be css:ssed tssuming th t tha TSPs are displaced in a SLB event to expose the entire length of the crack. Since the TSP prevents burst at normal operating conditions, a burst capability of 1.43APsts = 3661 psi satisfies the Regulatory Guide 1.21 burst margin guideline.

The crack length and average depth, together with the depth profile, provide the basis for estimating the free span burst pressure. The limiting crack is then R37 C28 wich has the longest length of 0.89" with an average depth of 54.8%.

Figure 1 shows th s + Point depth profile for R37 C28. The depth profile is fairly uniform, and the indication burst pressure would not be expected to be below that corresponding to the average depth. For a 0.89' long indication, an average depth of about 80% satisfies the 1.43APstsburst margin. The difference of 25% between the acceptable average depth and the NDE average depth of 54.8% provides an allowance for NDE uncertainty. Based on the EPRI Appendix H qualified depth sizing evaluation (Westinghouse Report SG 97 007, " Qualification of Rotating Coil Depth Sizing for Axial PWSCC at Dented TSP Intersections",2/97), the NDE uncertainty has a standard deviation of 7.3%

and uncertainty of 11.9% at 95% confidence. The measured average depth is less than the acceptable 80% depth by more than three standard deviations and satisfies the burst margin requirement. It is concluded that all axial PWSCC

?ndications at dented TSP indications satisfy Regulatory Guide 1.121 burst margin requirements even under the assumption that the TSPs displace in a SLB event to expose the entire crack length.

For SLB leakage considerations, maximum voltage and maximum depth orovide the best estimate of leakage potential. For dented TSP intersections, the length l of cracks outside the TSP would leak approximately as a free span indication, and the length inside the TSP would not contribute significantly to leakage. '

Since R14 C73 and R36 C42 extend outside the TSP and are among the largest voltage indications at locations outside the TSP, they represent limiting leak tests. If the plates were conservatively assumed to displace in a SLB event, they are still limiting cracks along with R 37 C28 (Figure 1). Tube R14 C73 has a maximum depth of only 76% and would not be expected to be through wall.

Tube R36 C42 (Figure 2) has a maximum depth of 91% with a single data point above 83% depth. The NDE uncertainty on maximum depth has a standard deviation of 10.2% and an uncertainty about 21.8% at 95% confidence such that indications would have to exceed about 80% depth to have a reasonable likelihood of being through wall and leaking Therefore, P14 C73 and R37 C28 would have a negligible likelihood of leaking at SLB condition while R36 C42 could potentially leak based on analyst results. All 3 of these indications were in-situ tested with no leakage up to 4750 psi. Since the deeper of two cracks in R36 C42 is outside the TSP (Figure 2), the in-situ test is a valid test independent of assumptions related to TSP displacement in a SLB event. SLB leakege would not be expected at SLB conditions even under the conservative assumption of TSP displacement in a SLB event. The in-situ test results support no or negligible leakage at SLB conditions.

Operational Assessment for PWSCC TSP The + Point inspection at EOC-8 of dented TSP intersections was slightly more extensive than that performed at EOC-7. All indications at dented TSP intersections satisfied burst and leakage requirements at EOC-8 and can be expected to satisfy requirements at EOC-9. It can be concluded that tube integrity requirements for axial PWSCC at dented intersections will also be satisfied at EOC-9.

Acceptable tube integrity at EOC-9 can also be obtained from considerations of

+ Point detection thresholds. An estimate of the + Point detection threshold for PWSCC at dented TSP intersections can be obtained as the smallest puiled tube indication that was detected in the field inspection. This detected indication can be reasonably expected to bound undetected indications. From Westinghouse Report SG 97-02-007, the smallest pulled tube indication had a l crack length of 0.12", a maximum depth of 38%, and an average depth of 23%. l A 0.9" crack would satisfy Regulatory Guide 1.121 requirements with an average i depth of 80%. It is very unlikely that an indication near the detection threshold I of about 23% average depth would grow to challenge tube integrity at EOC-9.

ODSCC TSP SON Unit 1 experience with circumferential ODSCC associated with TSP dents was projected to increase this outage based on industry experience. However, actual indications were lower than expected. This can be attributed to the effectiveness of chemical cleaning during the Unit 1 Cycle 7 outage.

A total of 32 circumferential ODSCC indications in 29 tubes were detected at dented TSP intersections. The longest circumferental crack was 102 (R5 C15 and R7 C11) in SG 4, which is below the 238 Regulatory Guide 1.121 structural limit. It is expected for circumferential cracks at dents to be self limiting due to the localized stress fields. Adequate detection of circumferentialindications at dented TSPs is supported by the prior tube pulls at SON Unit 1 and Diablo Canyon Unit 1.

A total of 301 axial ODSCC indications was detected in 279 tubes at dented and non-dented intersections during the Unit 1 Cycle 8 outage. APC implementation continued in non-dented tubes, and 263 tubes were left in service based on application of APC. A total of 3 tubes were plugged with ODSCC indications at dented intersections, and 13 tubes were plugged for other reasons (discussed in other sections of this report). A detailed report containing the 90-day reporting criteria for APC is Attachment 1.

- Mixed Mode Crackina .

' Mixed mode cracking was detected'in dented TSPs, and OD and ID cracking in the same intersection was also detected. Eight of the tubes were sized and considered for in-situ testing, and 3 tubes were chosen for in-situ pressure testing based on percent wall loss, axial crack length, and intersecting axial and t

circumferential cracks. These tubes were judged to bound the other indications.

No leakage was detected, and all three tubes exhibited a b_urst pressure abc e the requirements of Regulatory Guide 1.121. Details of the tubes in-situ teaad 4'

AVB Wear A total of 10 pluggable tubes were predicted for AV8 wear although SQN's experience has been below industry experience, and this mechanism has proved self-limiting.

A total of 69 indications in 37 tubes was detected with only one tube plugged; SG 4 R38 C54 at AV2+.00 with a 40% indication.

Cold L ea Wastaae A total of 10 tubes was predicted to be plugged for cold leg wastage. No pluggable tubes were detected from 16 cold leg wastage indications.

1.oose Parts Damaae Two tubes were preventively plugged in SG 2 for potential loose parts damage at the top of the tubesheet. No tubes were predicted to be pluggt;d for loose parts damage.

Other Pluaaed Tubes Two tubes were preventively plugged. SG 2 R28 C43 was plugged because this tube could not be inspected with available RPC equipment. SG2 R16 C22 was plugged because it was adjacent to a misplaced stabilizer from U1C7.

Cracked Support Plate indications Cracked tube support plate indications (CSis) are indications of cracks in the tube support plate and not necessarily indicative of tube degradation. A total of l

29 tubes were reported with CSis. There were 7 in SG 1,8 in SG 2,10 in SG 3 and 4 in SG 4. These were detected during an automated analysis of bobbin data and have been tracked at SON since 1990 and will continue to be tracked.

I No tube repair action was required.

SON Unit 1 steam generators do not have extensive support plate cracking.

Therefore, design basis function of the support plate has nc. been lost. There is also no evidence of wrapper drop or wrapper degradation.

CONCLUSIONS p

for Inservice Inspection; therefore, each SG has been demonstrated operable.
Alternate' Plugging Criteria was implemented in accordance with the Unit 1 Technical Specification License Condition 2.C(9)(d).

Based on ;he criteria of 10 CFR 50.59 and utilizing the criteria of Regulatory Guide 1.121, TVA concludes that the integrity of the Sequoyeh Nuclear Plant I' Unit 1 steam generators was maintained during Cycle 8 and will_ be maintained

through full cycle 9, and does not represent an unreviewed safety question.
