t l SQN STI-17 Page 1 Revision 2 1.0 OBJECTIVE AND RESPONSIBILITY 1.1 OBJECTIVE The objective of this Special Test Instruction is to demonstrate, by performance of DC high voltage tests, the integrity of selected representative cables installed in ,. conduits. The test is being performed to satisfy concerns identified by the Nuclear Regulatory Commission (NRC) in their Technical Evaluation Report (TER) on Sequoyah Nuclear Plant (SQh> cable pulling concerns. These concerns address the potential for cable damage due to the weight of cables being supported by 90* condulets or conduit tees. To demonstrate the integrity of the vertical cables, a worst case conduit and cable installation will have a dc high potential test performed on the cables in the conduit. The magnitude of the ac voltage test is in accordance with that specified for such testing in IEEE Standard 383-1974. The test program outlined in Reference 2.1.1 has been evaluated and been determined to have no short or long term effects on the cables or raceways involved. Successful completion of this test program on representative cabies will demonstrate the adequacy, with respect to the issues identified above, of all cables previously installed at SQN. The integrity of future cable installations will be ensured by the use of updated applicable j installation procedures. 1.2 , RE5PONSIBILIT The preparer of this special test procedure is designated as the' test director, having full responsibility for coordination at the test location and for the conduct of the test. The preparer is responsible for the technical adequacy of the special test instruction.' Coordination of the direction of the performance of this special test shall be implemented through the supervisory personnel of all groups involved.

A copy of the training roster (s) of personnel,who have been trained shall be attached to the Data Pac.kage. 4.0 PRECAUTIONS AND LIMITATIONS 4.1 The area arour.d the exposed ends of the cables under test must be roped off or isolated by means of orange tape and caution signs. 4.2 Only authorized testing and support personnel shall be allowed within the roped off test boundaries while the test is'in progress. 4.3 Personnel should.be stationed in the area of the exposed ends of the test cable while the test 1,s in progress to preclude any entry into the test boundary. 4.4 , Cable testing must be scheduled and coordinated so that at no time will both trains of safety related equipment (Train A and Train B) be placed in an inoperational condition. , 4.5 Any deficiencies found during the conduct of this test shall be 'j documented o,n the test Deficiency Log (Attachment 5). 4.6 Test equipment used in the conduct of this test shall be documented on the Special Test Equipment Log (Attachment 4). 4.7 The cables involved in.this test have asbestos braid jackets. .! Adequate precautions in accordance with HCI-HM4 shall be adhered to. 4.8 Other circuits in junction box 2271 and at valve 2-FCV-61-192 and 194 may be energized. Refer to Section 7.0 for acceptance criteria. 6.1.26 After completion of the five (5) minute test gradually v decrease the voltage level to zero, and deenergize the test set. 6.1.27 Install a ground lead on the test specimen and maintain the ground for a period of five (5) minutes to ensure all voltage charges have drained t,o ground. 6.1.28 Disconnect et'st leads and all grounds installed for the performance of this test. Repeat steps 6.1.17 through 6.1.28 for the remaining conductors. 6.1.28.a Reverify steps 6.1.1 through 5.1.15 to ensure that test status has not changed ar.d that all configuration changes are recorded on the Temporary Condition Log lI (Attachment 3). f NOTE: Inhalation of asbestos fibers may cause lung disease depending upon the concentration and size of the fibers, the type of fiber, and the exposure time. l~ DEFINITIONS [ , Action level - an airborne concentration of asbestos of 0.1 fiber each cubic centimeter (f/cc) of air calculated as an eight (8) - hour time-weighted average.
- Fiber -
a particulate form of asbestos, tremolite, anthophyllite, or actinolite 5 micrometers or longer, with a length-to-diameter ratio of at least 3 to 1.
"High-efficiency particulate air (HEPA) filter" - a filter capable of trapping and retaining at least 99.97 percent of 0.3 micrometer diameter monodisperse particles.
Permissible Exposure Limit (PEL) - an airborne concentration of asbestos of 0.2 fiber each cubic centimeter of air as an eight (8) - hour time-weighted average (TWA). PURCHASE OF MATERIALS CONTAINING ASBESTOS (1) Materials containing asbestos shall be replaced by asbestos free ' materials on a programmatic basis.

(2) The originator of a requisition or purchase order for asbestos or
- materials containing asbestos shall route such requisitions through the plant safety staff for review and approval.
(3). If no asbestos-free substitute is available for materials ecetaining asbest'os, the materialshall be properly labeled in its shipping _ container and when installed. PERSONAL PROTECTIVE EQUIPMENT (1) Employees exposed to airborne concentrations of asbestos shall wear j an approved respirator, protective coveralls, plastic booties, protective gloves and protective overshoes. All openings shall be taped shut. No personal outer clothing shall be worn under the protective clothing. Employees shall remove their protective

clothing and respirator prior to exiting the roped-off area. The asbestos contaminated clothing shall be bagged and handled in accordance with the requirements listed in the section entitled Laundering of Asbestos Contaminated Clothing of this HCI, Disposable booties and tape shall be handled as asbestos waste. (2) , Single-use or disposable type respirators (such as 3M Model No. 8710) are not approved for employees exposed to airborne concentrations of asbestos. LAUNDERING OF ASBESTOS CONTAMINATED CLOTHING Asbestos: (1) Contaminated clothing shall be laundered in a manner to prevent contact and airborne exposure to those doing the laundering. (2) Contract laundry establishments (if used) shall be informed of any _ abnormally high concentration of asbestos in clothing. .
