2 C. Concern JAM-86-001. Radioactive material is improperly stored in ,

lockers and cabinets in the auxiliary which are also not properly labeled.

O. Concern MRS-85 003. Green tags are no longer required for teols in the clean tool rcom. Green tags were used by the licensee to indicate that a particular tool had been surveyed by health physics and fcune uncontaminated.

III. Esaluation A. 00-85 005 011. TVA intervie ed personnel responsible for radioactive waste handling and release to the environment. They determined that curing the period 1981 through 1984, alum sludge from a water treatment plant at Sequoyah was hauled by private contractor tanker trucks and placed en fielcs as a soil conditioner. The water 1 treatnent facility from whicn the sludge was removed was separate fret any conta ,inated or potentially contaminated system. This

ractice was stepped in 1955 due to a change in State of Tennessee regulations. Botn sluege and soil samples had been taken and

, analy:ec ty TVA at various ti.tes during this operation.

E. .0 10'.-003. Such an event involving Na-24 on December 1,1950,

-as sa stantiated. Na-24 is used for moisture carryover testing curir; initial startup. There was no environmental or personnel ei;0sure pr0ble? A. 5 a result of the event due to the short half-life 1 of 'i3 24 TVA evaluated tne generic applicability of this event to a'atts Ear and Bellefente.

able to obtain ready access to locked containers since many were not latelee to indicate the responsible organization from which to obtain tre key. TVA took action to correct this pr:blem.

. N 5-55 CC3. 'anen Sequoyah changee to the ::.er bicek concept the restrictec area boundary was changed so that the clean tool room is re- cutside the restricted area. TVA HP personnel sursey tools at

e restricted area 0:undary so there is n: neec to tag tools in the tool roc. . Prior to the change, it was possible for a tool to te placed in the teol reem ithout going throug'1 the HP survey station.

Therefore, a green tag was required to sho that HP had surveyed the teol.
