Radioactive material is improperly stored in lockers and cabinets in the auxiliary which are also not properly labeled.

Green tags are no longer required for teols in the clean tool rcom.

Green tags were used by the licensee to indicate that a particular tool had been surveyed by health physics and fcune uncontaminated.

III. Esaluation A.

00-85 005 011.

TVA intervie ed personnel responsible for radioactive waste handling and release to the environment.

They determined that curing the period 1981 through 1984, alum sludge from a water treatment plant at Sequoyah was hauled by private contractor tanker trucks and placed en fielcs as a soil conditioner.

The water 1

treatnent facility from whicn the sludge was removed was separate fret any conta,inated or potentially contaminated system.

This

ractice was stepped in 1955 due to a change in State of Tennessee regulations.

Botn sluege and soil samples had been taken and analy:ec ty TVA at various ti.tes during this operation.

TVA did identify a problem with HP not being able to obtain ready access to locked containers since many were not 4

latelee to indicate the responsible organization from which to obtain tre key.

TVA took action to correct this pr:blem.
