!!!. Evaluation TVA has evaluated tre root cause of this concern to be the f ailure to ensure that Craf t for4*en receive the same training that is proviced the Craft personnel.

l The ime.ediate corrective action was to issue a Bulletin to each site identifying the danger of side pulls.

This information was disseminated to the operators, foremen. general foremen, and rigging personn:1 at Seavoyah in August 1986.

In addition, the Bw11etin stated that side owils w:vid net be used unless authori:ed by the Division of Nuclear Engineerieg.
