Based on the inspection conducted on September 11 and 14, 1978, it appears that certain of your activities were in noncompliance l with NRC requirements, as noted below. These items are deficiencies. l

1. 10 CFR 20.401(a) requires that you maintain records showing the radiation exposures on Form NRC-5, in accordance with the instruc- l tions contained in that form or on a cleat and legible record 1 containing all the information required by Form NRC-5. i Contrary to this requirement, on the day of the inspection, 1 September 11 and 14, 1978, your exposure records did not  !

contain the required information. Specifically, they do not 1 include birthdates and social security numbers for the personnel for whom such monitoring is required. <

2. Section D, Paragraph 5.1 of your Operating and Emergency Procedures specify that physical radiation surveys be conducted of the outer surfaces and passenger's compartment of transporting vehicles

' 'after the exposure device is secured and that a record of these surveys be maintained.

1 Contrary to the above requirements, records of vehicle surveys ,

were not being maintained. This is a repeat item of nonecmpliance. l l
