1 Gentlemen This refers to the inspection conducted by Mr. B. B. Dicey of this office on September 11 and 14, 1978, of activities at B&M We.iding
& Testing Company, Inc. authorized by NRC Byproduct Material License No. 24-17183-01 and to the discussion of our findings with you at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compli-ance with the Commission's rules and regulations and with the conditions of your license. The inspection consisted of a selectivo examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During this inspection, certain of your activities appeared to be in noncompliance with NRC requirements, as described in the enclosed Appendix A.
L The inspection also included a review of the actions described in your letter dated August 31, 1977, regarding the apparent items of noncompliance found during our July 22, 1977 inspection. We have no further questions regarding items 1, 2 and 4.
We noted during this inspection thc: item 3 (records of vehicle surveys not being maintained) had not been corrected in that such records ve.re not being maintained as of the dates of this inspection.
Thus, deficiency 2 in Append..t A to this letter is a repeat of the same noncompliance noted durn.g our July 22, 1977 inspection.
This notice is sent to you pursuant to the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title
- 10, Code of Federal Regulations. Section 2.201 requires you to submit to this office within twenty days of your receipt

2 of this notice a written statement or explanation in reply, including for each item of noncompliance: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
