The 332nd meeting of the Advisory Comittee on Reactor Safeguards, held at 1717 H Street, N.W., Washington, D.C., was convened by Chainnan W. Kerr at 8:30 a.m., Thursday, December 3, 1987

The Chainnan said that the agenda for the meeting had been published. He identified the items to be discussed on Thursday. He stated that the meeting was being held in conformance with the Federal Advisory Comittee Act and the Government in the Subshine Act, Public Laws92-463 and 94-409, respectively.

He also noted that a transcript of some of the public portions of the meeting l was being taken, and would be available in the NRC Public Document Room at 1717 H Street, N.W., Washington, D.C.

The Chaintan announced a change in the meeting agenda. He explained that the scheduled speakers were unavailable to describe nuclear indu*try activities to improve nuclear power plant operations, to resolve gP7diC issues more effectively and to interface with the NRC more effective 1). This session has been rescheduled for the ACRS January meeting, j Dr. Kerr said that the Comission has removed the stay previously imposed on Seabrook. This does not grant Seabrook an operating license but allows the Naring process to proceed.

Dr. Kerr said that a full power operating license has been issued for Palo Verde 3.

He advised the members that a television tape on Soviet PWRs was available for viewing during lunch breaks.

There was no word from the Comissinn on the naming of new members to the ACRS.

Mr. Ward said that he has arranged for a seminar on Human Factors on February 10, 1988. Mr. Richard Hughes will be the speaker. All members are en-couraged to attend, e

Mr. W. Kane, Director of the Division of Reactor Projects, Region I, said that the Peach Bottom Nuclear Plant was shut down by order on March 31, 1987.  !

In April 1987 the ACRS was briefed on the background of the reasons leading to the shutdown. Following the ACRS April meeting, the Comission was briefed. In May the NRC Staff met with the licensee on the status of their investigation. Since then the Staff has had a series of public meetings to get public input on the Philadelphia Electric Company (PECO) plan. About i one-third of the public coments were in favor of restarting the plant 1 quickly. About two-thirds expressed some problems with research, management, enforcement history, and the need for modifications to the Mark I contain-ment, i The approach for reviewing the licensee restart plan involves extensive involvement by the various offices of the Comission that led to the shut- I down. The approach used was to put together a panel to coordinate the Staff I review of the licensee plan and to develop an evaluation report to support a  ;

restart decision. l l

PEC0 has submitted a restart plan which the Staff characterizes as a laundry list of various activities that they felt they needed to accomplish prior to restart. Some of these pertained to the reasons for the shutdown order and some pertained to additional things they wanted to accomplish. The initial response of the Staff to the licensee's response was that they had missed what the Staff considered a fundamental point and that was the ability of the corporate processes to identify plant problems to senior management at the plant.

The Comission was briefed on the restart plan on September 15, 1987 The Staff noted the specific problems they had with the plan. The Com4sion responded to the licensee, noting that in their view the utility had a lot more work to do and that they were concerned with the lack of corporate involvement and the need to see some results.

The panel sent a letter to the licensee deferring review of the plan until they were able to address the fundamental root causes, to self-identify their problems, to bring them up through their system, and to get them resolved.

In response to this, the licensee has now submitted a plan which is called "Section 1. Corporate Action." This plan is intended to address the corpo-rate issue. Along with this there has been a major reorganization within the corporation to pull all of the nuclear activities under one senior vice president. The Nuclear Review Board has been expanded. Three individuals have been added from outside the comany.

Mr. William Regan, member of the NRC Peach Bottom panel, said that the Peach Bottom rehabilitation program is being evaluated for adequacy in terms of the i

0 332ND ACRS MEET!hG MINUTES 3 i material submitted and by observation of the training sessions. The opera-tors are being interviewed during their training and after the training is completed. The operators will be interviewed by psychologists to assess whether there has been a change in attitude. These evaluations will be an element in the decision of whether Peach Bottom is ready for restart.

Mr. W. Kane, Chainnan, NRC Peach Bottom panel, said that the decision to allow restart will be a judgment based upon the opinions of the best experts that the panel can bring to bear on the subject. One factor that is impor-tant is the comand and control of the shift, and the ability of the shift managers. Another important concern is the ability of the degreed shift managers to command the respect of the shift crews and to control the shifts.

This will be evaluated as the shift crews participate in simulater training. '

The evaluation will be by NRC licensed operator 6xaminers and resident inspectors.

The restart decision will be based principally upon three points: Can the utility in fact use the operators that were there under previous conditions?

Can the shift managers control the operators? Is there a mechanism in place to detect future problems? These will be the primary factors in arriving at the decision on whether to allow the plant to restart.

III.TVANuclearPowerPlantOperations(0 pen)

Mr. Wylie stated that the Subcomittee would be holding a meeting on the TVA >

management and Sequoyah restart issues in the near future and asked for ACRS input as to what issues should be discussed at this meeting or otherwise addressed in the future. The following suggestions were made:

1. The effectiveness of TVA's internal safety review process was  !

discussed at the November 4,1987 meeting of the Subcomittee and 1

, at the November meeting of the ACRS. The organization system l appears to be workable with proper staffing and management support. I The Subcomittee should ironitor future actions taken in this safety i review process as a means of evaluating its effectiveness. l

2. The adequacy of the electrical cable testing procedures and the resolution of the employee concerns as to the adequacy of the cable installation will be discussed at the next Subcomittee meeting.
3. The environmental qualification of electrical equipment will be discussed at the Subcomittee meeting.
4 The conclusion to be drawn from the Integrated Design Inspection (101) as to the effectiveness of TVA's "get well" programs will be discussed at the next Subcomittee meeting, e

, 5. The resolution of the individual technical issues raised in the IDI will be discussed. The design of pipe supports and the containment integrity during the design basis flood will be among the issues enphasized. t

6. The NRC Staff's capability for evaluating licensee management should be discussed in the near future. The TVA and NRC office of Special Projects issues will be addressed at the Subcomittee meeting.

The Subcomittee will bring its reconnendations and a discussion of the most important issues before the Comittee at the February 1988 ACRS meeting.

ACRS action is expected at this meeting.

IV. Quantitative Safety Goals (0 pen)

Mr. Ward, Chaiman of the Safety Philosophy Technology and Criteria Subcom-mittee, introduced the subject with some sumary coments of the Subcomittee meeting held on December 2,1987 to discuss the Staff's proposed implementa- i tion plan for the Safety Goal Policy Statement. The Staff's revised plan is

an effort to incorporate the Comittee's coments of May 13, 1987 which were ,

j generally endorsed by the Comission. Mr. Ward indicated that on the basis  !

of the material presented at the Subcomittee meeting, the plan is still in a i preliminary state and formal coments by the Comittee should be deferred until after a more complete version is available in late January 1988.

Mr. W. Houston (RES) covered four general areas in his presentation of the  !

Staff's proposed implementation plan. These were: (1) the three element plan l 1

recomended by the Comittee and endorsed by the Comission (2) a general 1

overview of the hierarchial structure. (3) some specific details for Level 2

- Quantitative Health Objectives (QH0s), and (4) some alternative consid-erations for Level 3 - Definition of a large Release. He indicated that the

Comission has requested that two option papers be issued, tentatively by i January 15, 1988
(1) Large Release Guidelines (Level 3), and (2) Plant i Perfonnance Objectives (Level 4).

Mr. Houston indicated that the primary goal of the Staff's plan would be to l address the adequacy of regulations, rules and regulatory practices, both i current and future. This was one of the Comittee's recomendations with a

proviso that the safety goal not be applied in making plant-specific regula-tory judgments. However, in Comission guidance provided on November 6, 1987, the Comission endorsed the consideration of that information as a 1

factor in licensing decisions.

The componegt goals for core melt (10g/Ry) and conditional containment failure (10" /Ry) were also discussed. Dr. Remick indicated that the safety goals include the risk from routine operations, not just accidents. Dr.

Lewis indicated that the specification of too many values had led to some ine and recomended that only one value (large release at 10"gnsistencies

/Ry) be adopted.

In regard to the definition of a large release Dr. Remick questioned the consistency of the proposed safety goal and health effects criteria. Dr.

Lewis indicated that he saw no need for a definition. Dr. Siess indicated an interest in the probability of a large release but not the magnitude. Mr.

Houston indicated that a number of the Staff agreed that the best thing to do is not to define a large release. He also expressed an uncertainty as to whether they could devise a meaningful definition. l Dr. Siess questioned the use of dose in defining risk from a large release.

He indicated that this was not specified in the Safety Goal Policy Statement.

Mr. Houston presented some data from accident sequence analyses for NUREG-1150 and indicated that PRAs will show some rather large releases (many calculated values larger by far than Chernobyl). Dr. Siess indicated that our resources have been put in the wrong place; they should be placed more on the probability of a large release rather than on the magnitude.

V. NRC Safety Research Program - Meeting with the Director of RES (0 pen / Closed)

During this pertion of the meeting, the ACRS met with Mr. Eric Beckjord, Director of RES, to discuss items of mutual interest. Sorre of the items discussed are given below.

A. RES Efforts to Accomooate the Ma.ior _ Reconsnendations of the National

Research Council (0 pen)

Mr. Beckjord stated that in response to a request from the NRC, the i National Research Council formed a Comittee on Nuclear Safety Research j to perform an independent study of the future role of the NRC safety  !

research. The results of this study are documented in a report enti- l tied, "Revitalizing Nuclear Safety Research." In its study, the Council Comittee placed major emphasis on research management issues rather than on technical issues.

Mr. Beckjord stated that NRC responses to the major recomendations of the National Research Council are delineated in SECY-87-271, dated November 2, 1987 4

i 332ND ACRS MEETING MINUTES 6 Some of the major recomendations of the National Research Council and the NRC efforts to accommodate them are given below.

. The NRC should b71ng in high-caliber people to bolster management.

Mr. Beckjord stated that in response to this reconenendation he tried to recruit sone highly qualified people from national labora-tories and universities; but all his efforts were in vain. An as alternate, he plans to bring in rome qualified people for six months or a year and let them review and provide recommendations on the technical and scientific aspects of the NRC research.

. The NRC should develop a cogent philosophy of safety research.

In response, Mr. Beckjord, with the help of Division Directors, has developed a research philosophy. One of the key points emphasized in the research philosophy is the importance of user offices in contributing to the development of research programs, and also to ensure that the research results will be useful in the reaulatory process.

. The research planning process should involve all relevant offices.

In response, they have established Senior Research Program Steering Groups, consisting of representatives from various NRC .0ffices.

These groups will, in their assigned areas of responsibility, focus their activity on evaluating the broad program plans developed within RES and provide written reconsnendations to the RES Director.

These groups will be supported by Research Project Review Groups consisting of members from Research user offices and the EDO,

. The NRC should empanel an independent advisory groun reporting to ,

In response, the Comission has approved the creation of an inde-pendent advisory comittee called the "Nuclear Safety Research Re- l view Comittee" (NSRRC). The objective is to have the NSRRC func- I tional by January 1,1988. The purpose of this Comittee is to make assessments of and recomendations concerning:

- Conformance of the NRC research to the NRC philosophy of nuclear safety research, and to specific Comission di-rections.

- Likelihood of the programs neeting the needs of the users of research.

- Appropriateness of the longer-range research programs and the correctness of their directions. l l

- Whether the best people are doing the work at the best places.

- Whether research products have been given adequate, unbiased peer review.

. The NRC should create a fair and competitive process for contract-  !

ing for research and should analyze the relative costs and benefits of consolidating work at national laboratories.

i . The NRC should do more work at universities.

Mr. Beckjord stated that the Strategic Plan states that the NRC will develop policies and procedures to enhance the NRC's capabil-ity to place research contracts at universities and other not-for- i

profit institutions on a timely basis. Those policies and proce- ,

dures will be utilized to create a fair and competitive process for contracting for research, thus allowing a better basis for analyz-ing alternative ways of conducting the work.

He said that based on discussions with other federal agencies, a i decision has been made to proceed aggressively with the use of the

! "Broad Agency Announcement" procedure of advertising needs for research support. Such announcements do nor preclude the participa-l tion of any research organization and have proved effective in i securing widespread university participation in support of agency programs. An estimated date for implementation of this procedure  !

d is January 1988 They expect that universities will respond to these announcements and compete favorably.

Dr. Remick coerented that RES should make sure that the Office of Contracts is aware of the above procedures. Also, it should be ensured that they encourage university participation. Mr. Beckjord responded that he does not foresee any problem in this area.

I . The NRC should institute an annual review of the pregram with the i principal performers of research.

Mr. Beckjord stated thct this reconenendation is reiterated in the Strategic Plan. The Office Director has met semiannually with the  !

principals at the major contractors. A meeting in the fall of 1987 1 was held to discuss:

- How to manage budget cuts.

- What safety research is needed that is not being done.

. The NRC should establish independent topical peer review groups and encourage publication of sponsored research in peer-reviewed journals.

In response to this recomendation, a policy his been established to restore the practice of using research review groups to review the work as it is planned and at key points in its perfonnance.

Also, a policy has been established that emphasizes the role of publication of research in the open literature in peer-reviewed journals. The major contractors have been encouraged to publish papers in mainstream scientific journals as well as in laboratory

reports and computer code manuals.

Mr. Ebersoie cocinented that the issue associated with the influence of control systems malfunctions on the performance of safety systems seems to have been overlooked in the NkC research. Mr.

. Beckjord agreed that it is an important issue and something needs to be d0ne. He said he would look into this matter.

B. RES' position on the Role of the ACRS in Reviewing the Overall NRC i Research Program and Budget (0 pen)

Mr. Beckjord suggested that the ACRS Chainnan and the Chainnan of the Nuclear Safety Research Review Comittee get together and cone up with an agreement regarding the roles of these two Committees in reviewing the NRC Safety Research Program and Budget so as to preclude possible  !

duplicate reviews. )

l Dr. Siess suggested that the ACRS continue to review particular aspects of the NRC Safety Research Program through individual subcounittees. He suggested also that, once or twice a year, RES provide an overview of the NRC Safety Research Program so as to rnable the ACRS to have a clear 1 perspective of the overall nature of the AC research.

Mr. Beckjord agreed with the suggestion. provided by Dr. Siess. He also stated that, in the future RES will be suggesting some areas for ACRS review, and ACRS may be interested in reviewing certain aspects of the NRC research program; to accomplish this, he suggested that the ACRS and RES meet periodically to come up with a list of research issues that should be reviewed by the ACRS and/or its subcorrnittees.

vidual subcomittees should look not only at the technical espects of the programs but also at the application of the research results.

C. Interaction Between NRR and RES Regarding Containment Performance Criteria (open)

Mr. Beckjord stated that he has been meeting periodically with Dr.

Murley, Director of NRR, to discuss containment-related issues. Dr.

Speis stated that RES and NRR had met once every two weeks for about four months to discuss issues associated with containments. In fact, NRR has appointed one person to work with RES until these issues are resolved. They have briefed the Committee to Review Generic Require-ments (CRGR) on the status of the resolution of these issues. RES has put together a Commission paper on this matter with NRR concurrence and sent it to the EDO. Questions raised by the ACRS, CRGR, Commission, and others related to containment issues are being looked at by RES and NRR.

They plan to provide preliminary recommendations on these issues in the spring of 1988 and final recommendations in December 1988 He believes that a close coordination exists between RES and NRR with respect to resolving containment related issues.

D. Impacts of the Anticipated Budget Reduction on the Continuing and Froposed Research Contracts (Closed)

1 VI. Advanced Light Water Reactors, (0 pen)

Mr. Wylie Chairman of the ACRS Subcommittee on Improved LWRs, reported that the Subcocaittee had net and reviewed Chapter 1 of the EPRI ALWR requirements document. Mr. Wylie reported *e 'he full Conmittee and the full Committee requested a presentation by EPR I Mr. Carl Stahlkopf, Director of the Materials and Systems Development Depart-1 ment at EPRI, has the overall responsibility for the advanced light water

reactor program. Mr. Stahlkopf introduced Ed Kintner, Executive Vice Presi-

, dent of GPU; Jack Devine, Program Manager, EPRIt Bill Sugnet, EPRl; and Dave McGoff of DOE.

Mr. Ed Kintner, Executive Vice President, GPU, noted that EPRI canvassed the i

nuclear utility on the subject of nuclear power about five years ago. There wasn't much enthusiasm regarding spending money on nuclear plants. There was a desire that some work should be ongoing to set the stage for future needs for nuclear plants and that an effort should be made to look at past experi-

) ence and to try to do better next time, i

, The ALWR program is funded at about $4 million per year by EPRI. There are '

! about 19 utility executives who review the work and try to provide guidance

] policy.

One of the objectives of the EPRI program is' to get a stabilized regulatory basis. It would be desirable to have an understanding with the necessary 4

332ND ACRS MEETING MINUTES 10 regulatory officials that what is being done is appropriate and correct and l can in fact be licensed with minimum difficulty. EPRI wants to establish a set of design and perforcance requirements (based on experience) which could be used with any supplier as a basis for the design and supply. An objective is to examine fundamental changes that might be made to achieve designs that might result in more reliable, more stable, and safer plants.

Future plants will be designed to have a significant safety advantage that can be demonstrated. One way of demonstration will be the use of a PRA. The PRA will be used in guiding the design. Future plants will be simpler.  ;

Capital and maintenance costs will be reduced. Design margins will be '

improved in terms of not pressing the technology to the absolute limits.

Proven technology will be stressed. Human factors will be considered. The operators will have longer time constants before any action is required. The ALWR people are working with the NRC Staff to select the important generic and unresolved safety issues to attempt to cone to closure on them as part of the ALWR program.

Mr. Kintner noted that they are planning to maintain larger inventories of water over the core in the reactor vessel. He cited the example of TMI-2 where in the areas of the core where there was water the fuel was essentially undamaged. He also noted the use of gravity for decay heat removal.

Mr. Stahlkopf noted that the program is focused on utility leadership. The program has three separate elements. The first is regulation stabilization.

The seccnd is the development of the utility requirements documents which are a set of comprehensive design requirerents for the advanced light water reactor covering both active and passive systems. The third is to bring the passive design concepts to a level of maturity that will allow vendors to request certification about 1990. Mr. Stahlkopf remarked that one of the najor challenges is tryir.g to understand how the current licensing regula-tions will apply to the new advanced plants, since they are so different than those currently licensed.

The active and passive designs will proceed in parallel. Mr. Stahlkopf noted that, although the passive design is a departure from the current active systems, they believe that large-scale engineering tests would be sufficient to confirm the designs without the necessity of a prototype.

There was a discussion on valve performance. The question was asked if EPRI was trying to design, specify or procure valves that are more reliable than those that exist in current plants. Mr. Stahlkopf noted that there was an effort under way on valve reliability which would be fed back into the program for procurement of valves. It was s99Cested that all the inforration on valves throughout the EPRI ALWR requirements document might be combined and presented in one section on va.ves.

Mr. Jack Devine Senior Program Manager, EPRI, discussed the ALWR utility requirerents document. He noted 13 chapters and a "roll up document" which will incorporate changes after all 13 chapters are reviewed. He noted some of the design criteria: 4

60-year lifetime. The steam generators will be designed to be replaced t

during the 60-year lifetime.

. Plant construction would be 54 months. Actual construction would be 48 months and startup and tet, ting would take 6 months. Ninety percent of the riesign would be completed before construction.

Mr. Devine discussed responses to the ACRS letter of Janucry 1987. He noted that the emphasis was to provide safety by minimizing the potential for upsets by defense-in-depth rather than providing protective features to deal with an accident once it happens. Regarding RHR, bunkered systems were not specified. A great deal of diversity and redundancy was specified. Spatial separation and physical separation are specified. Regarding the N plus 2 guidelines for safety train redundancy, Mr. Devine noted that they didn't

! specify this criterion. However, in many cases this turned out to be the case.

( Some of the Committee menbers discussed the rationale of two trains of power i supply. Mr. Devine noted that the current thinking was for two trains. The I section on electric power is not complete. Mr. Devine remarked that there might be two diesels plus something else. Regarding protection against sabotage, the primary focus will be to achieve and maintain safe shutdown ,

He indi-cated that a meeting of researchers, industry and the public would be held in late February 1988 to consider the issues. An interim report with reconnen- l cations to the Comission is tentatively scheduled for April 30, 1988 and a j final report for August 30, 1988 He requested ACRS review of the interim and final reports, as appropriate.

Mr. Ebersole questioned whether venting was being considered before or after I core damage. He also asked why the Staff had not alloweo a dedicated vent system to be installed at Pilgrim.

Mr. Hulman replied that venting was being analyzed under all conditions and that the Staff had not technically rejected the system at Pilgrim but had I requested more analyse; and information. He indicated that all BWRs had approved venting procedures as part of their Emergency Operating Frocedure (E0Ps). He corrected this later, under questionin had approved Emergency Procedure Guidelines (EPGs) g, to mean which would that the Staff be translated at the plant into E0Ps. He could not say if all plants had actual venting E0Ps or if they had equipment qualified to perform venting. He indicated that the Staff was preparing a White Paper on filtered venting.  ;

Mr. Houston indicated that the Pilgrim j request applied to a concept of venting after core damage while the current '

332ND ACRS MEETING MINUTES 13 Mr. Reed and Dr. Moeller discussed the Subcomittee meeting of Nove:ter 24, 1987. Mr. rem discussed readiness reviews applied to nuclear power plants. He stated that the paper trail does not ensure quality and that quality in a product must be built in. He l compared quality in the 1940's and 1950's in the United States as being high and Japan's low, and that currently the role it; re-versed. With respect to readiness reviews applied to Vogtle, Unit 1, he stated that Georgia Power Co., owner of Vogtle, and the NRC believe that the program was a success and provided significant added assurance that licensing comitments and NRC regulations have been met. In addition, he felt that the readiness review program can provide stability and predictability to the licensing process.

Dr. Moeller discussed readiness reviews applied to waste facili-ties. He stated that readiness reviews are required in the licens-ing of waste facilities and that DOE has used this or similar methods of quality assurance for man / years. For example, he discussed the use of readiness reviews as applied to BWIP. He stated that, because of quality concerns, DOE issued stop work orders on many operations at PWIP. Through tne use of readiness reviews these s')p work orders are presently being addressed and i some lifted.

Dr. Kerr said that so far seven members have submitted their list of important nuclear issues. Mr. Fraley has assembled the issues into 31 different topical areas. There are nine topical areas with concerns identified by more than one member. After discussion of how best to proceed, Dr. Kerr agreed that he would appoint a member to perform an hierarchical study of those important nuclear issues that have been identified by the ACRS members.

The Committee agreed that, unless committee members notify Mr.

Fraley of thcse nuclear power plants that they wish to adopt, it will be presumed that they are not interested in a particular assignment. (Note: The transition to this system is planned to occur on January 1, 1988.)

Mr. Wylie suggested that ACRS SubcomitLcts on Improved BWRs, 1 Improved PWRs and Improved LWR Cesigns be renared. The Comittee agreed. The new names are: Subcomittee on Advanced BWRs, Subcom- i mittee on Advanced PWRs, and Subcomittee on Improved LWRs. I Assignments, members, etc., of the previously named subcomittees will remain the same.

Review" (in Tab 6.1 of the Meeting Notebook) satisfied the sugges- l tion for such a report that was made during the Planning Subcomit-  !

tee Meeting, October 22-24, 1987, in Reston, Virginia. No one l indicated a need for the report. Because Dr. l.ewis had originally suggested such a report, he was asked to review the sample report to see if it satisfies his suggestion and if he believes the report is worthwhile.

The Comittee discussed the need for an educational seminar on containment venting and agreed thdt such a seminar is not now I needed.

3. Integrated Safety Assessment Program (0 pen)

Mr. Ward, Chairman for the Integrated Safety Assessment Program (ISAP) Subcomittee, began the discussion with reference to his l memorandum to all members dated November 23, 1987 and asked if the ,

l Comittee should contrent on the matter of promoting the ISAP process for future application.

Mr. Reed indicated that the NRC Staff should state up front the inducements for a utility to choose the ISAP process. He also indicated that utilities were ir, undated with requests which detract from the nonnal operation of the plant and he questioned whether thers was Regulatory acceptance of prioritized items.

Dr. Siess reviewed briefly the Systematic Evaluation Program (SEP) and ISAP and indicated that til of the review items of the two programs can come together in the resolution of Generic Issues. He also indicated that utilities didn't like to comit to an ISAP schedule since any changes to the schedule would require a license amendment.

Mr. Cappucci (NRR) indicated that the license would only cover the l comitment for fixing, not the schedule.

Dr. Kerr questioned whether ISAP would contribute significantly to safety and suggested postponing action until further meetings were held or more information was provided by the utilities.

Mr. Ward moved that this matter be given to the Generic Issues Subcomittee for further action. The Comittee agreed.

4. Decay Heat Removal Systems (0per)

[Mr. P. A. Boehnert was the Designated Federal Official for this portion of the meeting.]

Mr. Ward, Decay Heat Removal Systems (DHRS) Subcomittee Chairman, reported on the results of the November 17, 1987 DHRS Subcomittee meeting. The meeting was held to discuss the following items:

a. The decision by Toledo Edison (TE) not to install a dedicated blowdown system at Davis Besse.
b. Implications of secordary side water level control, vis-a-vis operator actions, in B&W OTSGs in accident situations.
c. Implications of the Diablo Canyon loss of shutdown cooling event vis-a-vis a lack of steam generator water box vents.

These issues had been raised by Mr. Reed over the past four months and the ACRS asked thu DHRS Subcommittee to explore them in more detail.

Regarding item a., Mr. Ward noted the details of the June 9,1985 loss of feed event at Davis-Besse (DB). He noted that DB doesn't have high-head HPCI pumps that would allow "true" feed-and-bleed (F&B) cooling (i.e., pump against the PORVs and/or code safeties).

Toledo Edison explored the option of adding additional blowdown capacity to increase the reliability of the F&B cooling option.  :

Analyses conducted by the utility showed that such a modification could result in core uncovery during the blowdown process. In-stead, Toledo Edison decided to add an additional makeup pump to  :

the one available so that they can pump against the PORV. This i upgrade, however, is not fully safety-grade. NRR has not completed its review of this modification, but Mr. Ward noted that NRR has no requirements in this area as F&B is not a design basis requirement.

In response to Dr. Shewmon, Mr. Ward said NRR's review will focus ,

on not making the plant less safe with this modification. In l response to Dr. Remick, Mr. Ward said Toledo Edison only comitted I to analyze installation of this system. Mr. Ward noted that Mr.

Reed objects to Toledo Edison's approach and has drafted a letter detailing his concerns.

Mr. Reed outlined his concerns with the current TE approach to F&B cooling. Essentially, Mr. Reed objects to increasing RCS pressure to the PORY setpoint in order to begin F&B cooling. The trend of i increasing pressure in such a situation runs counter to an opera- l tor's "comon sense." It also is a challenge from the standpoint l of PTS, SG tube integrity, and PORV reliability, etc. '

There was discussion of Mr. Reed's concerns. Some members ques-tioned the validity of his concerns. Mr. Ebersole said there are i

332ND ACRS MEETING MINUTES 16 questions outstanding regarding the ability of the operator to understand the plant's status once F&B is undertaken vis-a-vis the process instrumentations available to him.

Dr. Kerr asked Mr. Reed what his recommendation would be here. Mr.

Reed indicated that the Comittee should express concern with the DB F&B approach for the reasons cited above.

Concerning item b., Mr. Reed indicated that for the low-set OTSGs on relevant B&W plants, the operator, depending on the accident at hand, may have either to raise or to lower the secondary water level to assure core cooling. Mr. Michelson said that the B&W plant Accident Transient Operator Guideliness tell the operator to fill the SG to the 95% operating range and keep it there. Mr. Reed said he believes the situation is too complex for an operator to cope. Mr. Michelson said the issue of the validity of the operat-ing procedures needs to be factored into this area. As an example, Mr. Michelson said the operator will be "blind" when the water level in the vessel falls (as B&W plants do not have a vessel water level system).

In regard to item c., Mr. Reed said the Diablo Canyon loss of RHR event shows a significant design deficiency with current PWR U-tube plant designs, given a lack of SG water box vents. He said that there are methods ava'ilaHe to circumvent this problem and NRR seems to have the situation in hand. He has written a close-out memorandum that proposes some follow-on action vis-a-vis the

. Staff's proposed requirements for half-locp operation. Mr.

Michelson proposed that the Committee look into the topic of accident analyses for plant operations during shutdown. There was some discussion about which subcommittee should be assigned this task. Dr. Kerr said that this assignment will be considered at a future meeting of the Management Subcommittee.

Regarding Mr. Reed's proposed letter, Mr. Ward indicated that the DHRS Subcommittee didn't see a need for further action. Mr.

Michelson saw the need for additional examination on some of the points raised by Mr. Reed. Mr. Ebersole said there are some concerns with the validity of the code results for F&B operation.

Dr. Kerr asked what action the Comittee wished to take. C.

Michelson proposed that a subcomittee explore the issue of F&B in more detail. Dr. Kerr indicated his support of this motion. Dr.

Siess proposed that F&B be explored as a potential generic isst.e.

Mr. Ward reported on the activities of the Thermal Hydraulic Phenomena (T/H) Subcommittee during its meeting of November 18-19, 1987. He said the Subcommittee is in the process of reviewing the NRC-RES T/H research program with the goal of developing recommen-dations for Committee discussion and a Report to the Commission /

Congress. Another Subcommittee meeting is scheduled for January 20-21 to continue this review. The Subcommittee expects to bring this matter to the ACRS for its consideration at its February and/ l or March 1988 meeting.

Dr. Remick reported the New Member Subcommittee concern with the l dwindling size of the Committee and its recommendation that the i ACRS not fall below 13 members. He recommended that the Committee ask the Commission to appoint three new members without delay and recommended five candidates for addition to the two names sent to the Commission in September 1987. Af ter discussion of the recom-mended candidates, the Committee voted to propose three new names to the Commission for membership and to recornend that the Commit-tee's membership be maintained at 13 persons as the minimum number required to carry out its statutory respcnsibilities.

The Committee expressed its concern regarding the possibility of embrittlement of pressure vessel supnort structures. It recommend-ed early effort be made to determine 1) if any power plants are now operating with temperature of the pressure vessel support structure below its nil ductility transition temperature, and 2) if this condition will be reached in any operating plant before its license expires.

2. ACRS Comments on the Revised ASB 3-1 of Standard Review Plan 3.6.1, Requirements for Arbitrarily Postulated Jet Impingement Effects in the Break Exclusion Zone (Report to the Execut' ve Director for Operations,datedDecember8,1987) i

3. Safety Implications of Control Systems (0 pen) 1

Mr. Ebersole presented a draft ACRS letter comenting on the NRC Staff's resolution of USI A-47. Mr. Ebersole indicated that the NRC Staff has reached resolution of this USI by proposing certain requirements on 1) steam generator and reactor vessel main feedwater overfill protection, 2) automatic initiation of emergency feedwrter on the Oconee units, and 3) certain investigations and possible improvements to CE plants with low head injection pumps.

These matters will be submitted for public coment in March 1988 and the final resolution will be issued in April 1989.

Mr. Ebersole indicated that he concurs with the proposed Staff requirements, but believes that it is only a partial resolution of the problem. Dr. Lewis presented alternate paragraphs to the draf t letter stating that transients of the sort that occurred at Rancho Seco, Crystal River, Three Mile Island, and Chernobyl all featured instrumentation and/or control system involvement, and do not fit well into the Staff's treatment of this issue. ,

Mr. Ebersole indicated that the interfacing aspects between this USI and other USIs (such as A-17, A-46, and A-49) are not clear.

Dr. Siess comented that the relationship with other USIs e.g.,

A-17, A-46, A-49, should be integrated rather than interfaced. Dr.

332ND ACRS MEETING MINUTES 19 Siess also commented that the ACRS letter should be more specific and should mention the items that the ACRS would like the NRC Staff to resolve. Dr. Lewis in his second alternate paragraph stated that the Staff's resolution to this USI is in fact only a small, tentative beginning, and it does not seem to have been given either sufficiently high priority or sufficiently expert attention. Mr.

Ebersole agreed to redraft his letter with more specific items of concern. The Committee agreed to continue its discussion of this matter during its January 1988 meeting.

4 DOE Advanced Reactor Severe Accident Program The Committee agreed that the assignment of a subcommittee to follow the review of DOE's Advanced Reactor Severe Accident Program (ARSAP) should be deferred until after a December 16, 1987 meeting of staffs of NRC and 00E. The Committee will discuss this assign-ment during the January ACRS meeting.

5. Diablo Canyon Long Term Seismic Program The Committee agreed to the reauest of Pacific Gas and Electric Company for a one-year delay in the completion of the Diablo Canyon Long Term Seismic Program. (The completion date for the seismic program is now to be scheduled for July 1989.)
6. Diagnostic Evaluation Program The Committee agreed that the NRC Diagnostic Evaluation Program should be followed by an appropriate subcommittee. [This appears to be the Subcommittee on Systematic Assessment of Experience.] A subcommittee meeting will be scheduled when the NRC Staff's evalua-tion of the McGuire Plant is available.
