1.0 INTRODUCTION

The ourpose of this procedure is to ensure that once an item has been inspected and accepted, any subsequent work on the item will q
be identified and documented so that the reworked item may receive g
the required inspections. Specific exceptions to this procedure w
are given in paragra:,h 3.0.
m The use of the Inspected Item Removal Notice (IRN) form (figure 1) is necessary only when the rework is not identifed in some other mtuner or is s'pecificaly required by other procedures.

example, rework required by process control sheets such as travelers.
l weld data cards, termination cards, hanger modification packages,
i etc., or rework to the item because of design changes, NCR's, or "unsat" inspection reports, does not require an IRN to be prepared.
g The process control sheets may be new, or revised by adding additional holdpoints which require " repeat" inspections of the reworked items.
If however, another item needs to have work
-g performed on it to permit the working of the af fected item, an IRN is required for the other item.
IRN's are for cocumenting the removal of an inspected item and shall not be used'as a design change authorization.

PROCEDURE EFFECTIVE CPSES NUMBER REVISION DATE PAGE CP-CPM 6.10 11 02/16/84 2 of 4 2.0 PROCEDL'RE 2.1 IRN PREPARATION Except as noted above, the IRN shall be initiated, normally by the responsible craft foreman, or his designee, when it is determined a previously inspected item has to be removed or partially dis-assembled. Only the craft responsible for the installation of the

item may prepare an IRN or remove the item.
If another craft's work 1
must be removed, notify the appropriate craft and request removal of the item.
The following is an explanation of various parts of the form.
1.
" Location" - Sufficient information shall be given to provide for timely location of the af fected item.

" Item Description" - Provide adequate description of the item

to be removed. EXAMPLE: Hanger #AF-1-101-032-S53R.

" Identification", Provide suf ficient information to uniquely l
identify the item.
EXAMPLE: Hanger MK-315.

" System / Structure Designation" - Record the system / structure designation sufficient to identify the item location within a system or structure, if known.
EXAMPLE: Volume Control System.

" Reason for Removal" - Explain briefly the reason for removal of the item.
EXAMPLE: Interferes with the installation of cable tray.
2.2 IRN APPROVAL AND ROUTING

After preparation, the IRN shall be submitted to the responsible 4
superintendent for approval (" Responsible superintendent" as used here is normally the discipline superintendent. However, approval i
by the " night" or " weekend" general superintendents is also acceptable. Where an area is under the control of an area lead,
-1 only the area lead =ay approve the IRN for vaultad Ranger Packages.
The area lead will approve the IRN only af ter the package af fecting-the item has been presented.) After approval by the superintendent, i
the IRN shall be presented to the discipline IRN coordinator.

4.0 VOIDING IRN's

Strict accountability of IRN's shall be maintained.
If an IRN g
is initiated and not required because of some change in the work N
scope, the IRN shall be returned to the coordina tor and voided.
Q-Voiding an IRN may be acc omplis hed by preparing a supplementary M
'RN which explains the voiding of the original IRN, or by indicating on a carbon ;r xerox copy of the IRN being voided that it was voided.
In the latter case, the person responsible
-h for voiding the IRN shall sign and da te the voided copy.
Be M-g coordinator shall then void the entry in the log, and transmit E
the voided IRN to the applicable package flow control group 6,
