3.0 INSTRUCTION 3.1 INSPECTION TIMING Separation inspections shall be implemented upon notification from the building management organizations or the TUGC0 Site QA Supervi sor or his designee.

Continuation Sheet (Figure 3).

The conduit numbe r, branch circuit number, or tray node point shall be li sted on the Inspection Report Continuation Sheet.

l NOTE:

Unit 2 cables that are installed in Unit 1 and conmon buildings shall be installed and inspected by the applicable Building Matrix Organization.

The documentation shall be processed by the applicable PFG in accordance with Unit 1 criteria'.

3.3 DISCREPANCIES When unsatisfactory conditions are identified, the respective inspection attribute (s) on the IR shall be checked "Un-sat" and the deficient items and conditions shall be clearly and concisely recorded on a Electrical Separation Deficiency Repo rt, Figure 2. in accordance with instructions li sted on Figure 1.

The Deficiency Lists shall be attached to each respective IR and the IR's shall be processed in accordance with Reference 1-C.

Separation deficiencies may be reported on a NC R, Reference 1-0, if conditions warrant, or as directed by the QA/QC Supervisor.

3.4 Deficiency reports previously issued prior to Revision 14 of this Instruction shall be processed in accordance with Figure 5.

CPSES QI-QP-11.3-29 15 JAN 13 534 3 of 11 FIGURE 1 The following steps will be followed in processing of the Electrical Separation Deficiency Report (Figure 2) for Unit 1.

Step 1:

QC will report the unsatisfactcry as-found condition in the Black 2 Description Section and attach a " Separation Violation" Tag with the IR ntsnber and Deficiency Report item number entered on the line provided - (Figure 4).

The tag shall be placed at the area where the violation occurs.

2:

Paper Flow Group (PFG) will retain a copy of the Deficiency Report (with the. original IR) ano fonvard the original Deficiency Report to Engineering for disposition in Block 3.

3:

Upon canpletion of the Engineering Di sposition Block 3. Engineering will assign the responsible craft in Block 4,

and fonsard the original Deficiency Report to PFG for distr-ution.

NOTE:

When Block three (3) is di spositioned "Use as is" and the deficie y is not in accordance with ES-100, or the isposition is

" Rework" but the work is not the scope of CPP procedures, a HCR will be issued, and the deficient item closed referencing the NCR.

DCEPTION:

When a traveler is issued for rework, it is not necessary that a proced'ure be issueo. The traveler should have detailed instructions..

Engineer Review / Approval shall be by the Engineering Di scipline Engineer or his designee (Block 5).

Quality Assurance revi ew/approvel shall be by the Electrical Di scipline Quality Engineers or the Building QC Supervi sors designated to review / approve NCR* dispositions (Block 5).

After approval of the waiver, the QC Inspector shall check the waiver approved block and sign and date the appropriate section of the

" Separation Violation" Tag.

When the responsible craft canpletes the itens listed on the Deficiency Report, by working the Engineering dispostion and QC has signed all steps for acceptance, the QC Inspector shall remove the

" Separation Vi olation" tag.

of H FIGURE 5 The following steps will be followed in processing of Electrical Separation Deficiency Reports issued prior to Jan. 1,1984.

Step 1:

Inspection Reports shall be issued, one per room / area.

The Separation-Deficiency Report shall be attached to the IR.

Step 2:

Paper Flow Group (PFG) will retain a copy of the Deficiency Report (with the original IR) and "

forward the original Deficiency Report to Engineering for disposition in Block 3.

Step 3:

Upon completion of the Engineering disposition, Engineering will assign the responsible craft (Block 4) and forward the original Deficiency Report to PFG for distribution.

Step 4:

When the responsible craft completes the itens Itsted on the Deficiency Report, PF' will be notified by memo with the Deficiency Report IR number and iten nLaber to be inspected.

Step 5:

PFG will notify QC for inspection.

Step 6:

QC will verify that the itan has been capleted.

If the item is found to be acceptable, the QC Inspector will sign and date Block 5 of the Deficiency Report.

If the itan is found to be unacceptable, it will be noted on the mmo from Construction and be returned to the originator by PFG.

Step 7-Upon completion of the Separation Deficiency Reports, the IR shall be closed and processed in accordance with Reference 1-C.

Step 8:

For those unsatisfactory conditions w ch would restrain or impact ongoing construction.:tivities, Engineering shall evaluate the conditions and may g

request authorization to proceed according to the following:

The QC Supervisor shall be contacted and requested to transfer the separation deficiency to the. new Separation Deficiency Report Fom (Figure 2).

b.

The Separation Deficiency and Engineeri ng Disposition shall be transferred fra the old Deficiency Report Fom (Figure 6) to the nos report fom (Figure 2). The QC Supervi sor or his designated Inspector shall sign and date block 2 on the new report fo ne.

The designated Engineer shall sign and date block 3 on the new report fom.

The next sequential Deficiency Item ntaber of the Separation Deficiencies under the respective Inspection Report shall be assigned to the transferred deficiency and entered in block 1.

EXAMPLE: Unc.- IR No. XXX, 39 Separation Deficiencies were reported and docunented on the old Deficiency Repe ' fonns (Figure 6).

Then, Itar 10 shall be assigned to the defis ency transferred to the new report fona.

d.

The transferred Separation Deficiency on the old report fann shall be closed with the following note:
