e APPENDIX A NOTICE OF VIOLATION Florida Power and Light Company License Nos. DPR-67 CPPR-144 F

Based on the results of the NRC inspection conducted on May 15-18, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in our correspondence to you dated December 31, 1974.

A.

Criterion V of Appendix B to 10 CFR 50, as implemented by the FSAR, paragraph 17.1.5.1 and paragraph 5.2.1 of the FP&L QA Manual requires in part that, " Activities affecting quality shall be prescribed by documented instructions, procedures,

. and shall be accomplished in accordance with the instructions, procedures..

Paragraph 5.2 of FP&L Quality Instruction QI 10-PR/PSL-3, FP&L procedure for the control of maintenance work, requires in part that, " Applicable PWO shall i

be

prepared, reviewed and processed according to AP #0010432..." and " Inspection hold points, examination requirements and examination acceptance criteria shall be inserted by QC, during review by this procedure;" In addition, paragraph 5.6 of this QI requires in part that, " Examinations shall be documented by the inspector performing or witnessing examinations on a nondestructive examination report form QI 10-PR/PSL-3-1.

This report shall contain the following information.

Contrary to the above requirements, on May 16, 1978, the inspector found that for PT inspection of repairs to mounting flanges for Unit 1 safety relief valves MS 8201 and 8202, the examination requirement and examination acceptance criteria had i

not been entered on the PWO paper work at the time of the inspection.

In addition, report form QI 10-PR/PSL-3-1 was not filled out after the inspection.

This is a deficiency.

B.

Criteria V of Appendix B to 10 CFR 50, as implemented by FP&L Topical Quality Assurance Report, Section 5.1 specified, in part, that activities affecting quality are prescribed and to be accomplished in accordance with documented procedures.

FP&L Procedure ASP-3, Rev. O specifies the procedure for material

Purchase order numbers were not written on pending inspection labels attached to reinforcing steel having Material Receiving Report (MRR) numbers 08946 and 08975 located in the auxiliary building staging laydown area and for pipe spool pieces found near field storage area N-7 identified by MRR No. 08754. This is in nonconformance with FP&L's Procedure ASP-3, paragraph 8.2.4.2.

Warehouse G-2 was found to contain three or four pallets of water-stop material which was placed in a pending inspection status.

However, only one pallet of the group was properly tagged with a pending inspection tag. This is in nonconformance with FP&L's Procedure ASP-3, paragraph 8.2.4.2.

Examination of records revealed that two reels of copper grounding wire (MRR No. 00246) should have been placed in a impending inspection status during the interim between off-loading at the warehouse and QC inspection. However, the tag (or label) numbers of the impending inspection tag were not recorded on the MRR form as required by FP&L's Procedure ASP-3, paragraph 8.2.4.2.

Warehouse G-1 was found to contain several boxes of Boric Acid Concentrator (a conforming item) that did not have proper identification on them in that the quality group designation was missing on the boxes.

This is in nonconformance with FP&L's Procedure ASP-3, paragraph 8.2.4.7.

This is an infraction.

C.

Criterion V of Appendix B to 10 CFR 50, as implemented by FP&L Topical Quality Assurance Report, Section 5.1 specified, in part, that activities affecting quality are prescribed and to be accomplished in accordance with documented procedures.

CEND-353, Rev. 6, paragraph 4.2.17 and Side 2 of Attachment No. I to Procedure ASP-17 " Preventive Maintenance Requirements" specifies a two week shaft rotation frequency for the subject motor to preclude bearing damage.

Contrary to the above requirements, review of preventive maintenance records for Unit 2's safety injection pump motor (MRR No. E-00100, P0 No. CE-13172) revealed that the subject motor had been receiving monthly shaft rotation since the program starting date (February 2,1978).

This is an infraction.
