SUBJECT:

TRIP REPORT - DAVIS-BESSE On February 7-8, 1985,. Messrs. N. Jackiw, W. Rogers, D. Kosloff and I met with Toledo Edison Company management to discuss,the status of the licensee's Performance Enhancement Program (PEP) and the status of the Action Plan being used to improve their overall training effort.

Mr. J. McMillen, DRS, also attended the meeting relating to the training program.

As a result of the Performance Enhancement Program effort the licensee has developed fifty-five Action Plans to address known problems that will improve their performance in various areas. A copy of the Action Plans is attached for your review.

Each Action Plan item was discussed with members of management (or their alternates) directly responsible for the implementation of the plans. These discussions revealed that the key personnel were knowledgeable of their assignments and that progress was being made towards resolution of the issues.

We intend to continue to monitor the status of all the Action Plans; however, some of the plans are more important to regulatory performance than others and we intend to request that the licensee incorporate these more important plans in their next response to the SALP IV report. The Action Plans that we con-sider to be less important are indicative, however, of a concerted effort by the licensee to identify and resolve their problems before they become regula-tory issues.

With regards to the Action Plan to improve overall training, discussions revealed that some progress was being made.

However, the licensee has not identified a sufficient number of milestones that would allow us to track their progress. There appeared to be some hesitancy on their part to make specific commitments. After discussing this issue Mr. T. Murray assured me he would develop the necessary milestones and provide them in the same format as the PEP Action Plans by March 8. 1985.

For planning purposes the licensee has agreed to meet on the first Thursday of each month until we feel that these meeting frequencies can be reduced or i

terminated. The next meeting is tentatively scheduled for March 8, 1985, i

I am convinced that these people are sincere about improving their regulatory per-formance but they have not yet established that they can take the initiative and move forward on their own without our nudging.

The Action Plan for overall training improvements is an example of a lack of initiative. The plan has existed longer than the PEP Action Plans yet we had to push them to provide adequate milestones - something they should have been working on at least a year ago.

The objective of this inspection is to assess how' key management personnel connunicate ongoing station activities with their supervisors, peers and

' subordinates. This assessment is intended to provide Region III some insight into Davis-Besse's inability to communicate ongoing problems to the highest level in the organization.

Each Davis-Besse supervisor shall be the escort for the assigned NRC 1

team member.

4.

Each team member shall remain with the assigned Davis-Besse person for the entire workday and will observe and note the following:

a.

Meetings (1) attendance i

(2) what is communicated b.

How, if and when the individual communicates with his subordinates i

c.

How, if and when the individual communicates with his peers i

d.

How, if and when the individual communicates with his management i

e.

Particular attention shall be paid to what information is transmitted to the individual and how promptly action is taken f.

In each instance, note the time the individual was informed and 3

the time the individual informed someone else i

At 3 p.m. on each day the team will debrief at the DBAB and collectively determine what information should have been reported to the Toledo Edison Directors and the Mission Head.

At 4:30 p.m. on each day the team members will meet with Davis-Besse Directors and Mission Head. The Directors and Mission Head will provid'e a summary of all issues that were communicated to them during _.

the day. Each NRC team member will determine if these individuals have been promptly informed and how aware they are of ongoing site activities.

The 4:30 p.m. meeting assignments are as follows:

In discussions with Division Heads:

Let them tell you what was communicated to them b.

Find out who made the contact and what time the contact was made c.

Determine if any direction was given by the Mission Head or Director i

Did individual receive any information that could impact the plant and was it transmitted to the site? How?

On Thursday the Team shall meet with C. E. Norelius for a briefing of ongoing activities for the week.

9.

C. E. Norelius will meet with Vice President Crause to determine how knowledgeable this individual is of ongoing activities at Davis-Besse.

10. On Friday, C. E. Norelius will conduct a fonnal exit with the licensee to sumarize the findings of the Team.

NOTE:

At any convenient time during this effort each Team Member will solicit the assigned individual (s) opinion on what that person perceives to be the major problem with comunications in the TECo organization.

Also, the Team member will determine if this perception was solicited or passed on to Senior Management.
