-4 1.0 PURPOSE 1.1 This procedure is to govern the inspection and reporting of concrete placement, curing and finishing.

2.0 SCOPE 2.1 This procedure covers inspection of placing concrete frpm the placing contractor's receptacle to the place of final deposit and the placing, curing, protecting, repairing and finishing of the concrete. It also m covers the clean-up and other preparation of the areas in which the 2:

concrete is to be deposited.

E 2.2 The forms, reinforcing steel, releasing agent, tie rods, embedments and

$l other concrete placement preparations are to be inspected in d accordance with Procedure No. QCIP-6.

M 4.1 Inspector - In this procedure the " Inspector" shall mean the E person responsible to the Quality Control Civil Supervisor and who

$ shall assure that the points outlined in this procedure are followed

4.2 Specifications - Where the phrase " specifications" is used in this procedure it shall be understood to refer to specifications which

are included on the construction drawings or in Specification LOU-1564.472 - Concrete Masonry. ~

4.3 Aggregates " Aggregate" designates both the fine and coarse inert materials used in the manufacture of concrete. Sand is a fine aggregate. Crushed rock, natural gravel, or crushed gravel are coarse aggregates.

i 4.4 Drawings - Drawings in this procedure shall mean design drawings.

NO.QCIP-7 ISSUE: 3 WATERFORD STEAM ELECTRIC STATION - UNIT NO 3 PACE: 2 OF 6 4.5 Receptacle " Receptacle" as used in this procedure shall mean the concrete hopper, concrete bucket and/or form when discharging concrete directly into forms.

5.0 RESPONSIBILITY 5.1 The Quality Control Civil Supervisor is responsible for the inspection and the recording of the results of the inspection of the items covered by this procedure.

5.2 The Inspector (s) report to the Quality Control Civil Supervisor and g shall be responsible for performing the inspection of the work g covered by this procedure.

E 6.0 PROCEDURE us

    • t 6.1 Inspection of Placing Equipment to h 6.1.1 Before concrete placing begins, a review of the placing program g shall be made consisting of equipment to be used and methods e of placing. The appropriate information will be entered on g Form QC-20 by the Inspector. ,

.= 6.1.2 The Inspector shall ensure during placing that the Contractor S observes the following limits imposed by the Specification:

The information shall be recorded on Form QC-20, under the

}y appropriate item or remarks.

$ 6.1.2.1 When using concrete bucket that it is spotted over g the point designated for discharge.

z 6.1.2.2 Unconfined height of drop not to exceed 5 feet.

with a " Positive" method of regulating the amount and y rate of concrete deposited.

6.2 Ordering Concrete 6.2.1' The Inspector shall insure that the Concrete Preplacement Checklist Record (QC-34) and the Concrete Preplacement Inspection Checklist QC-76 have been properly filled out and signed.

WATERFORD STEAM ELECTRIC STATION - UNIT NO 3 PAGE: 3 0F 6 6.2.2 The Inspector shall make a final inspection of the placement to insure that the surface against which concrete is to be pla::ed was maintained in a clean bondable condition, free of water, debris and other material. Record this on Form QC-20.

6.2.3 The Inspector's responsibilities concerning the ordering of concrete are limited to the followin'g:

6.2.3.1 He shall notify the Batch Plant Quality Control Inspector that the placement is " ready for conctete" when the items under 6.1 are complet,ed.

hi 6.2.3.2 He shall give the Batch Plant Inspector a location

{ and description of the placement, the mixes to be used and the approximate yardage of concrete to complete h the placement. This information is to keep the batch m, plant inspector informed.

N g 6.2.3.3 He shall notify the Batch Plant Inspector when a change in mix is required. This is for information only.

h 6.2.3.4 He shall insure that the Contractor is using the concrete z mixes specified on the drawings and specifications in u

, the designated locations of the strucedre.

g. 6.2.3.5 He shall advise the Contractor of an unsatisfactory y rate of placing.
  • s-l 6.2.3.6 If cold joints occur, the Contractor, upon the approval g of the Quality Control Civil Supervisor or his designee, m will be required to terminate placing activities and 5 prepare the surface in acceptable condition for sub-g sequent cleaning and resumption of placing concrete.

,. 6.2.3.7 Results of this inspection shall be reported on Form

e g 6.3 Receiving Concrete E! 6.3.1 The Inspector shall ensure that c'ontrol is maintained on the h consistency of the delivered concrete. This control shall

<: be by means of slump, temperature air meter and unit weight h tests. Sampling and recording shall be done by the Materials Imboratory Techniciah using Form QC-32.

6.3.1.1 Samplir.g and testing will be as follows:

a) Slump - each batch b) temperature - each 50 cy.

c) Air content - each 50 cy.

I d) Unit weight - each time cylinders are cast 6.3.2 'The Inspector shall ensure that the concrete placing Contractor does not add water or change the concrete in any way. The addition of water to concrete in a truck mixer shall be in accordance with the Spacifications and shall be approved by the i ,

Inspector.

. WATERFORD STEAM ELECTRIC STATION - UNIT NO 3 PACE: .4 0F 6 6.3.3 The Inspector shall ensure that concrete which has become stiffened so that it cannot be placed in satisfactory condition except by the addition of water is not deposited in the work.

6.3.4 The Inspector shall ensure that the slumps are maintained within the range listed in the Specifications.

6.3.5 The Inspector shall receive the truck ticket from the transporter and time stamp the ticket at the discharge point.

If the difference in time between the injection of the mix water and discharge point is not in accordance with the Speci-fication time limit the Inspector shall waste the concrete.

u l' 6.3.6 The Inspector shall ensure that the Form QC-20a is properly filled out by the Materials Laboratory Technician.

a 6.4 Placing Inspection i

m l 6.4.1 The Inspector shall ensure that the following inspection controls are observed and record this information on Form QC-20 h under the appropriate item or under remarks.

6.4.1.1 Concrete is placed in horizontal layers in progressive

, fashion from one side or end of the placement to the other.

u 6.4.1.2 Concrete is properly compacted by vibrators.

E 6.4.1.3 Vibrators are used in a vertical position.

d l 6.4.1.4 Vibrators penetrate the layer being placed and H

penetrate the layer below.

6.4.1.5 Care is taken to minimize contact of Vibrators against reinforcement steel as detailed in the Speci-

,. fications.

E' H 6.4.1.6 Care is taken to minimize contact with forms.

w 6.4.1.7 Care is tak'en to prevent segregation of concrete 2

when placing through reinforcing steel.

O g 6.4.1.8 That the Specifications are observed when placing g concrete walls with openings, in floors with integral slab and beam construction and other similar construction.

6.5 Finishing Concrete 6.5.1 The Inspector shall ensure that finishes shall be as required

' on Drawing and in Specifications. Record on Form QC-20 and Form QC-10.

6.6 Curing 6.6.1 The Inspector shall ensure that curing conditions as listed on Drawing and in Specifications are followed. Detailed descriptions of curing requirements and methods are listed in 9 the Specifications. Record Form QC-20 and Form QC-10.

Record on Form QC-10.

6.8 Protection 6.8.1 The Inspector shall ensure that all concrete is protected against damage until final acceptance by the g Owner. Record on Form QC-10.

6.9 Repair and Replacement of Unsatisfactory Concrete 5

g 6.9.1 The Inspector shall ensure that unsatisfactory concrete y is cut out and replaced with new concrete g or other means of patching in accordance with the

- Specification. Record under remarks on Form QC-20.

E u 6.10 Clean Up

U 6.10.1 The Inspector shall ensure that wash water is removed in a g manner to prevent running down and staining of concrete g surfaces which will be exposed at the completion of the job.

p 6.10.2 The Inspector shall ensure that concrete splatter is g removed frora any permanent equipment before it sets up.

z g 6.11.1 The Inspector shall ensure that forms are removed in a manner and at the time interval according to the Specifications.

E-6.12.1 The concrete placement record shall be recorded on Form No.

QC-20 and QC-20a. Form QC-20 gives identification of E placement, date, weather, ambient temperature, checkhst and

other information concerning the total placement. Form QC-20s is a detail record of each truck load. QC-20 will be filled out by the Inspector. QC-20s will be filled out by the Materials Laboratory Technician.

6.12.2 Concrete Physical Tests made at the place of discharge from the mixer, or at the pump discharge for the first 1000 cubic yards placed if a concrete pump is used, and the :esult a are entered on Form QC-32.

6.12.3 A record of curing, protection, form removal and surface preparation shall be kept on Form No. QC-10 for each placement..
