1.0 Purpose
To specify the serveillance and inspection activities that will will be performed by J. A. Jones Quality Assurtnce personnel to assure that the Concrete Mud Slab is installed correctly.
- 2. 0 Scope:
This procedure covers the Plteing, Curing, Finishing and Repair of the Concrete Mud Mat for Waterford SES Unit No. 3, Phase I Concrete Construction.

4.0 Responsibilities

- 4.1 Ebeoes is responsible for furnishing all materials, receiving inspection, procedure approval, testing and documentation of such test as required by 4.1 to support J. A. Jones in the performance of the work.
4' 2 J. A. Jones is rdirsponsible -for forming, placing, curing, finishing and visual inspection of the Concrete Mud Slab in accordance with " Approved for Construction" drawings and specifications.

FINISHING AND REPAIR.
PROJECT TITLE:
WATERFORD UNIT # 3 CONTPJLCT NO. W3-NY-4 6.1.1 Assure that the Engineer (Ebesco) has accepted the the placement of the clam shell filter blanket.
6.1.2 Assure proper that concrete is discharged from the mixer withIn 60 minutes or 300 revolutions of drum which ever occurs first. The time must not exceed 1% hours in any case.
6.1.3 Assure that concrete is placed in accordance with reference 3.2.

6.1.4 Assure that mud slab thickness is nominally 2"' thick.
x 6.1.5 Assure that.the mud slab elevation is within the tolerances specified on the " Approved for Construction" drawings (i % inch).
6.1.6 Assure proper curing of mud slab.
7.0 Records and Documentation:
The assigned Quality Assurance Inspector (s) shall perform the inspections listed in paragraph 6.0 and record the information on Attachment No. 8.1 " Mud Slab Inspection Report", Form No.
W-SITP-16.1, 8'. 0 Attachments:
