6.3.2 Form Installation - The inspector shall ensure that the form material and method of construction are satisfactory.

S 6.3.2.5 Form Oil - Form surfaces shall be clean and oil y treatment made before placement of any m! reinforcement steel adjacent to the forms. Excess oil on forms and oil on embedments shall be removed.

6.3.2.6 , Chamfers and Fillets - Moldings shall be provided, to produce chamfers or fillets on all exterior g corners and angles in the exposed structure and 3 on interior corners when specified.in the 8 drawings or by the Engineer.

k- 6.3.2.7 Vertical Construction' Joint Chamfers - On concrete H surfaces which will remain exposed after erection g of the structure, the construction joint edges shall be trued with a shaped edging strip that g

will provide a slight chamfer at the joint edges.

.M 6.3.2.8 Form Joints - Joints in forms shall be horizontal

( or. vertical unless otherwise specified and shall be smooth, tight and unyielding. Forms placed against j

existing concrete shall be fitted tight without gaps and braced to preclude bulging and mismatched surfaces.

6.3.2.9 Reuse of Forms - Used forms.shall be repaired to provide smooth and mortar-tight faces. Faces shall be cleaned and reoiled before the succeeding placement.

6.4.5 Mechanical Splices - The inspector shall ensure that splicing of bars are made only at locations shown on the drawings. Mechanical splices (Cadweld) will be inspected in accordance with QCIP-9,and the inspector shall ensure that all mechanical splices have been accepted prior to placement of concrete.

6.5__ EMBEDDED ITEMS _

a. The inspector shall ensure that anchor bolts, drains, pipe sleeves, piping, condult, wiring, flashing, manhole cover frames, suicps, instruments and all other embedded items are supported and firmly braced or tied before concrete is D placed. Preparation of contact surfaces depends on whether

i 6.6 Documentation ,

6.6.1 Quality Control Documentation - A document titled  ;

Concrete Preplacement Checklist Record, Form No.

QCIP-6-1, shall be provided by the Quality Control Civil Supervisor. This card shall be maintained near the pIacement to provide a constant status .i check for concrete placement preparations and is ,

a Quality Control document; it shall be signed by the' Quality Control Civil Supervisor when he is satisfied that concrete placement may proceed.

shall be denoted by marks in the N/A column. Items which are applicable but which were not inspected shall be E identified by notation "Not inspected" in the. Remarks column.

4 E 6.6.3 The Inspector shall report inspection discrepancias in accordance with ASP-III-11 to the Quality Control Ni Civil Supervisor.
