, 1.0 PURPOSE 1.1 This procedure is intended to be instructions for those Quality Control inspection functions which are required before placement of concrete to ensure that a quality product is obtained. , .
e 2.0 SCOPE 2.1 This procedure covers the inspection and check-off of na the condition of the excavation, existing concrete surfaces j against which concrete will be placed3 forms, placing of reinforcing steel,and placing of embedded items prior to final clearance for concrete placement.

6.1 Excavation and Foundation
. ~6.1.1 Dimensional _ Location of Excavation - The Field Engineer shall verify that location of the excavation has been checked.
6.1.2 Elevation of Principal Base Area - The Field Engineer shall verify that the subgrade elevation, drainage slopes, and surface shapes have been checked. . ..

6.1.3 Condition of Soil Ease Preparation - The inspector ;

6.2.2 Surface Preparation - The inspector shall ensure that concrete surfaces have been scarified to expose aggrega:e 8 for approximately 1/4 inch with all laitance, glazed m- mortar, dirt and unsound concrete removed.

6.3.2.1 Bracing and Rigidity - The Field Engineer shall evaluate the integrity of the formwork installation to ensure position and shape during concrete

6.3.2.2 Form Material - Form faces for surfaces that will remain exposed shall be made of metal, plywood, or sized and surfaced lumber or other g approved material.

- a. The inspector shall ensure that anchor bolts, drains, pipe sleeves, piping, condult, wiring, flashing, manhole cover l frames, sumps, instruments and all other embedded items )
i- are supported and firmly braced or tied before concrete is placed. Reinforcing steel shall be shifted or bent to clear embedded items. Preparation of contact surfaces depends on whether or not bond with concrete is required in accordance with drawings and specifications.

i 6.6 Documentation (1 6.6.1 Quality Control Documentation - A document titled Concrete Preplacenent Checklist Record, Form No.
QCIP-6-1, shall be provided by the Quality Control Civil Supervisor. ,This card shall be maintained near the placement to provide a constant status check for concrete placement preparations and is a Quality Control document; it shall be signed by the Quality Control Civil Supervisor when he is satisfied that concrete placement may proceed.
