Re: Renon on Non Imnlenwntatinn of Onerntnr Reanalificatian Plan for Dr. P. J. Rogers In the course of review of records in conjunction with an on-site inspection by Region IV inspector Blair Nicholas, the following omissions were found:

No annual written or pmetical examinations were conducted for Senior Operator Patricia J. Rogers between 7/662 and 2/1365 in violation of the requirements of the requalification plan for the facility for annual examinations. Dr. George Miller, the only other licensed operator (Senior or otherwise) at the facility is exempt from examination requirements as the Training Supervisor.

Dr. Rogers continued to operate the reactor between 7/662, when a full examination by NRC was l

administered, which she passed with excellent performance in all areas, and 2/8S4 at which time the reactor was shut-down for instrumentation upgrade work. This work was more prolonged than anticipated and extended until February 1995. She continued to perform maintenance and surveillance activities for the shut-down reactor, but no operation was done or was possible.

l During restart of the reactor during the period 2/12S5 thmugh 2/15S5, she was examined in both written and oral modes and reviewed all new and former practices and procedures before being returned to full status as an SRO for the facility.

Thus Dr. Rogers was an " active" Senior Operator (actually inserting the key and operating criticality contmis) for a period of about 19 months after her last exartunation. She then was an operator of a shutdown facility for a further period before being retrained to operate at facility restart.

Pronosed Com.ctive Actinn: steps will be taken to create a checklist for such items as required exam schedules to be reported regularly at ROC meetings, so that deadlines and frequencies are noted at semi-annualintervals.
