Date of Occurrence 10/22/78 Time of Occurrence 0620 Unit 1 Identification and Description of Occurrence: -

During normal operation, the unit operator observed an abnormal sample flow annunciation. Both main ptack offgas sampling pumps were inoperable..

Conditions Prior t6 Occurrence:

Unit 1 - 50% power

Unit 2 - 100% power Unit 3 - Refueling outage in progress Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

None required. ,

Apparent Cause of Occurrence:

Both vacuum pumps failed to operate.

i Analysis of Occurrence:

With both vacuum pumps out-of-service, the stack gas released was not monitored.

No change in plant conditions occurred during the time the monitors were not 4

available. and release via the stack was approximately the same after and before the loss of the monitors.

Corrective Action:

Replaced vacuum pump in one channel and replaced a motor on the vacuum pump in the redundant channel.
