I' An NRC resident inspector observed bolts missing on the unit 1 RCIC turbine exhaust rupture diaphragm assembly. A subsequent inspection' by plant engineers revealed that the outer rupture diaphragm was missing. The RCIC was declared inoperable on March 17, 1982 at 1710 hours0.0198 days <br />0.475 hours <br />0.00283 weeks <br />6.50655e-4 months <br /> to install an outer rupture diaphragm. The diaphragm was installed Jmd the RCIC was declared operable on March 17, 1982 at 1850 hours0.0214 days <br />0.514 hours <br />0.00306 weeks <br />7.03925e-4 months <br />.

• An investigation of this event revealed that when a modification to replace the inner rupture disc was performed, the outer rupture disc was not in place. The work instruction provided no written work instructions and the drawings included in the work instructions did not reflect the configuration of the inner and outei rupture discs. Normally the rupture discs were changed out as routine maintenance and field services personnel had little experience with the evolution, and due to lack of adequate documentation failed to install an outer rupture disc.

Since this work was performed, much more emphasis has been placed on reviewing workplans for adequate work instructions and references.

Mechanical Maintenance Instruction 22 will be revised to clarify-installation instructions for the RCIC rupture disc and will be revised by August 1, 1982.
