No. 23 Reactor Coolant Pump Flow Channel III - Inoperable.

This report was initiated by Incident Report 82-194.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 82% - Unit Load 870 MWe.

DESCRIPTION OF OCCURRENCE:

At 1420 hours0.0164 days <br />0.394 hours <br />0.00235 weeks <br />5.4031e-4 months <br />, July 24, 1982, during routine operation, the Control Room Operator observed that No. 23 Reactor Coolant Pump (RCP) Flow Channel III indication was erratic.

Channel III output appeared to be periodically drifting low and tripping the low flow bistable.

The channel was declared inoperable, and Limiting Condition for Operation 3.3.1 Action 7 was entered.

All bistables associated with the channel were immediately placed in the tripped condition.

The redundant No. 23 RCP Flow channels were operable throughout the occurrence.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The erratic operation of No. 23 RCP Flow Channel III instrument was due to a failed ferrite slug in the flow transmitter.

ANALYSIS OF OCCURRENCE:

The Loss of Flow trip is designed to insure a minimum DNBR of 1.30 during all normal operations and anticipated transients.

Since the redundant channels were operable and capable of providing this protection, the occurrence involved no risk to the health or safety of the public.

The event constituted operation in a degraded mode permitted by a limiting condition for operation, and is reportable in accordance with Technical Specification 6.9.1.9.b.

Limiting Condition for Operation 3.3.1 Action 7 requires:

With the number of operable channels one less than the total number of channels, power operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

CORRECTIVE ACTION

As noted, the bistables were placed in the tripped condition, in compliance with the action statement.

The failed transmitter was replaced with a spare, and the channel was satisfactorily recalibrated.

At 1525 hours0.0177 days <br />0.424 hours <br />0.00252 weeks <br />5.802625e-4 months <br />, July 27, 1982, No. 23 RCP Flow Channel III was declared operable.

However, on July 26, 1982, No. 24 RCP Flow Channel II had been declared inoperable for planned maintenance, and the action statement was still in effect.

With the return of No. 24 RCP Flow Channel II to operability at 1345 hours0.0156 days <br />0.374 hours <br />0.00222 weeks <br />5.117725e-4 months <br />, July 28, 1982, Limiting Condition for Operation 3.3.1 Action 7 was terminated.
