, SPECIAL REPORT 93-04 LOOSE-PART DETECTION SYSTEM CllANNEL INOPERAHl.E FOR GREATER TIIAN 30 DAYS This report is submitted in accordance with Technical Specification 3.3.3.8 Action (a) which states, "With one or more Loose-Part Detection System channel (s) inoperable for more than 30 days, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next 10 days outlining the cause of the malfunction and the plans for restoring the channel (s) to OPERABLE status."

At 10:00 CST on 11/19/93, Channel'7 of the Loose-Part Detection System was determined to be inoperable. The problem was originally thought to be in the charge converter located inside containment outside the bioshield. The plant was in Mode 3, Ilot Standby. The charge converter was replaced, but it did not eliminate the problem.

By that time, criticality had been achieved.

The Channel 7 accelerometer is located in the containment mounted on the tubesheet.

flange of Steam Generator "B". Further troubleshooting of Channel 7 has determined that the problem lies with the sensor mounting or associated sensor cabling. Since the instrument sensor is connected to a steam generator (thus inaccessible during power operations), further troubleshooting will not be performed until the next outage of sufficient duration. The remaining eleven channels in this system remain operable.

Until further troubleshooting has been completed, actions to be taken to prevent recurrence cannot be determined.
