Core end of life is near. During this power run, the plant has experiences some problems with the fuel and coolant activities are high. Also, there has been difficulty maintauung water inventory.

Several OSTs have been performed on this shift - the water balance OST results are due; the #2 diesel generator failed its OST due to a breaker _

problem. #1 DG is available. #C charging punp is out of service for corrective maintenance M1ile awaiting repair parts.

Just prior to 0730 the "B" steam generator develops a 10 gpm tube leak.

This results in increasing air ejector activity. At 0800 the results of the OST are available, and this data indicates leakage greater than tech-nical specificaitons. A shutdown is required in 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />, but, the shutdown is not started at the request of the system dispatcher, W1o is having grid load difficulties. Plant operators declare an unusual event on the basis of the forced shutdown and on the basis of the leakage.

At 0830 with reactor power at 100%, an Operational Basis Earthquake (OIE) occurs. Offsite power is lost. With the loss of load, the turbine trips and the reactor trips. The trenor increases the steam generator tube leak to 150 gpn and causes a 50 gpm RCS to containment leak. #1 DG starts arxl picks up loads properly. The charging pump maintains reactor pressure. On one of its transients, the code safety on the "B" steam generator sticks midway, such that the steam flow matches the incoming RCS leakage flow. An Alert emergency is declared.

At 1100 the BCS leak increases to 450 gpn. The charging pump is unable to keep up with the leak and reactor pressuer decreases rapidly to saturation.

Safety injection and containment isolation occur. A Site Area emergency is declared.

The diesel is tripped, resulting in a station blackout. Firefighters are called. A General Emergency is declared. 'Ihe fire is quickly extinguish-ed, but #1 DG will be out of service. Mamtenance reports that #2 DG will be available shortly.

Without cooling, the core begins to overheat, resulting in fuel damage.

Offsite power is finally restored at 1400 and appropriate measures are taken to regain control of the RCS pressure and to increase the cooldown rate to minimize the release from the affected generator.
