Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/91019 Technical Specification Involved: Surveillance Requirement 4.6.1.1.a Reported Under Technical Specification: 6.9.1.13.c Date of Occurrence: 2/11/81 Identification and Description of Occurrence:

During an inservice inspection, it was discovered that inside containment isolation valve, 1-33-704, on the service air header had been omitted from SI-14 which fulfills surveillance requirement 4.6.1.1.a. This system is not seismically qualified and does not contain any automatic valves for containment isolation.

Conditions Prior to Occurrence:

Unit in mode 5.

Apparent Cause of Occurrence:

A reissue of the drawing failed to indicate the valve to be closed and

• therefore the valve was omitted fr'on SI-14.

' Analysis of Occurrence:

A~ modification replaced the 1-33-704 check valve with a manual diaphragm valve.

Subsequent reissue of the drawing did not indicate the valve to be closed and therefore the valve was not included in SI-14. The outside isolation valve, 1-33-740, was included in SI-14 and requires the valve to be closed.

Corrective Action:

Valve 1-33-704 has been added to SI-14 and requires the valve to be closed.

This discrepancy also existed for valve 2-33-122 and has also been corrected.

Operations is presently reviewing the SI to ensure this condition does not exist for other valves.
