Tennessee Vc11ty Authrrity Sequoyah Nucicar Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/ 8038 Technical Specification Involved 4.4.7 Reported under Technical Specification 6.9.1.13.b Date of Occurrence: 4/14/80 Time of Occurrence: 0700 EST Unit 1 Identification and Description of Occurrence Surveillance Instruction SI-50 was initiated on April 13, 1980, but not all data was taken as required by Technical Specification SR 4.4.7. At ,

0700 EST 4/14/80, the lead chemical analyst discovered, when reviewing I completed surveillances performed for previous day, that the required chloride and fluoride analyses on reactor coolant were not performed. ,

He immediately had a sample taken and analyzed. The chloride and fluoride I concentration limits of Technical Specifications were met.

Conditions Prior to Occurrence Unit 1 fuel loading complete and in Mode 6.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. __

Not applicable.

Apparent Cause of Occurrence Personnel error.

Analysis of Occurrence There was no hazard to the public health or safety.

Corrective Action Reinstructed personnel on the importance of performing and completing surveillance instructions as scheduled.
