Control rocn integrity was breached and beth trains of control roce emergency air cicanup made inoperabic due to the removal cf centrol room door C-49 and removal of tape frem doors C-37 and C-50.

Conditions Prior to Lecurrence:

Reactor in Mode 3 with RCS Tavs = 547 F, Pressure = 1725 psig Apparent Cauce of Occurrenec:

Control z>cm daor C-49 was removed from its location to perfern maintenance en the latching rechanism. Control reem doors C-37 and C-50 had tape (used to prevent air inicakege) removed to allev access to the C3.

Analynis of Occurrence:

This occurrence brecched the entrol race integrity and made both trains of the enntrol room optrgency ventilation system inoperable.

Correcttve Actioq:

Door C-49 w.n replaccd at 0825 CDT, and doors C-37 aad C-30 were retaped to prevent air inleckage.
