Tennessee Valley Authority Sequoyah Nuclear Plant LER SUPPLEMENTAL INFORMATION SQRO-50-327/ 8029 Technical Specification Involved 3.6.5.9 Reported Under Technical Specification 6.9.1.12.1 Date of Occurrence 4/15/80 Time of Occurrence 11:30 EST Unit 1 Identification and Description of Occurrence:

During final inspection, prior to entering Mode 4, of the Containment Barrier Seal, holes in the flexible material were found which affect the ability of the seal to perform its intended function. Two areas along the wall were missing clamps. Many clamps had loose nuts holding them in place. Some caulking was damaged and leaking.

Conditions Prior to Occurrence: ,

Unit 1 in Mode 5.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment.

N/A Apparent Cause of Occurrence:

Apparently holes were made with a sharp tool. Unknown cause for missing clamps and loose nuts.

Analysis of Occurrence:

Deterioration is not due to aging.

Corrective Action:

All damaged areas will be replaced and new seal spliced at the ends. Missing clamps will be replaced. All nuts will be torqued to specifications. Caulking will be repaired as needed.
