MIDCYCLE PERFORMANCE REVIEW AND INSPECTION PLAN - ARKANSAS NUCLEAR ONE

Dear Mr. Forbes:

On August 4, 2004, the NRC staff completed its midcycle plant performance assessment of Arkansas Nuclear One. The midcycle review for Arkansas Nuclear One involved the participation of all technical divisions in evaluating performance for the first half of the calendar year 2004 assessment cycle. The process included a review of performance indicators (PIs) for the most recent quarter and inspection results over the previous 12 months. The purpose of this letter is to inform you of our assessment of your safety performance during this period and our plans for future inspections at your facility so that you will have an opportunity to prepare for these inspections and to inform us of any planned inspections that may conflict with your plant activities.

This performance review and inspection plan does not include physical protection information.

A separate letter designated and marked as Exempt from Public Disclosure in Accordance with 10 CFR 2.390 will include the physical protection review and resultant inspection plan.

Plant performance for the most recent quarter was within the Regulatory Response column of the NRCs Action Matrix based on a finding with low to moderate safety significance (White) that was issued on April 7, 2004, for failure to comply with the requirements pertaining to fire protection safe shutdown capability. A supplemental inspection to review the root cause, extent of condition, extent of cause, and corrective actions for this finding will be performed in September and October 2004. This supplemental inspection will be conducted in conjunction with the scheduled triennial baseline inspection of your fire protection program.

In our annual assessment letter dated March 3, 2004, we advised you of a substantive crosscutting issue in the area of problem identification and resolution. This substantive crosscutting issue was identified on the basis of a number of corrective action findings.

Throughout the 2003 assessment cycle, inspectors identified several (Green) findings which had crosscutting aspects of problem identification and resolution. These findings were

Entergy Operations, Inc. indicative of implementation problems within the specific areas of identifying problems and entering them in the corrective action program (CAP), prioritizing and evaluating conditions in the CAP, and implementing effective corrective actions. These problems were identified across cornerstones and with multiple groups of plant personnel.

At your request, a public meeting was held in the Region IV office on February 12, 2004, to discuss your actions to address the causes and corrective actions you had taken or planned in response to the CAP programmatic and implementation problems. Additionally, your letter dated July 28, 2004, detailed several corrective actions which have been implemented to improve performance. Through the conduct of baseline inspections, we have noted that these actions have resulted in improvements in the ANO staff's effectiveness in identifying and entering problems into the CAP. However, during the past 6 months we have identified isolated examples involving the adequacy and timeliness of corrective actions. As a result, the NRC plans to continue to focus on and assess the effectiveness of your actions in response to this substantive crosscutting issue, particularly within the areas of prioritization and implementation effectiveness, during the conduct of baseline and supplemental inspections.

The enclosed inspection plan details the inspections, except those related to physical protection, scheduled through March 31, 2006. Included in this plan are inspections associated with Unit 2 license renewal activities and Unit 1 reactor vessel head and steam generator replacements. The inspection plan is provided to minimize the resource impact on your staff and to allow for scheduling conflicts and personnel availability to be resolved in advance of inspector arrival onsite. Routine resident inspections are not listed because of their ongoing and continuous nature. The inspections in the last 12 months of the inspection plan are tentative and may be revised at the end-of-cycle review meeting.

In accordance with 10 CFR 2.390 of the NRCs Rules of Practice, a copy of this letter and its enclosure will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRC's document system (ADAMS).
