This refers to the routine safety inspection conducted by Messrs. T. M. Tongue, M. J. Farber, and W. J. Kropp of this office on February 2, 1986 through March 29, 1986, of activities at the Braidwood Station, Units 1 and 2, authorized by NRC Construction Permits No. CPPR-132 and No. CPPR-133 and to the discussion of our findings with Mr. M. Wallace and others of your staff at the conclusion of the inspectio The enclosed copy of our inspection report identifies areas examined during the inspection. Within these areas, the inspection consisted of a selective examination of procedures and representative records, observations, and interviews with personne The evaluation of the incorrect positioning of Safety Injection System valves and the improper application of caution tags during the performance of the Unit 1 Reactor Coolant System hydrostatic test was completed during this inspection period. This issue was initially addressed in Inspection Reports No. 456/85057(DRP) and 457/85053(DRP). As a result of our evaluation, we conclude that this matter represents a violation of NRC requirements. NRC Enforcement Policy, as stated in 10 CFR 2, Appendix C, provides an option for not issuing a Notice of Violation if the event meets all of the five stated criteria. Our evaluation of this event and the five factors is contained in the attached report. Due to your self-identification of this problem and the initiative which you have demonstrated in correcting the problem we have determined that no Notice of Violation will be issued. Notwithstanding, this decision should not be misconstrued to mean that the seriousness with which we view this event is diminished. This event represents a failure to properly execute two processes (system lineups and tagging) fundamental to the operation of a power plant and the results of your corrective action will be closely monitored during future inspection In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC Public Document Roo PDR ADOCK 05000456 G =PDR
