Licensee Action on Previous Inspection' Findings (Closed) Violation (456/84044-05; 457/84040-05): QA inspectors failed to identify areas where Phillips-Getschow Seismic Category I and other pipe support restraints have not been constructed in accordance with design requirement The NRC inspector reviewed the licensee's response to this violation dated May 16, 1985, and verified the licensee's corrective actions taken to prevent recurrenc The NRC inspector reviewed the current procedure for this activity wnich is Phillips-Getschow Company Procedure QCP-B23, " Installation and Inspection of Component Supports," Revision 14. The procedure was in conformance with ANSI N45.2.8," Supplementary Quality Assurance Requirements for Installation, Inspection and Testing of Mechanical Equipment and Systems for the Construction Phase of Nuclear Power Plants."

In addition, the NRC inspector reviewed documentation traveler packages produced under the QCP-823 procedure associated with 15 randomly chosen pipe supports which were recently QC inspected and accepted. The pipe support packages reviewed were as follows:

Also, the hanger installation and adjustment checklists utilized to document in-process QC inspections were examined in the traveler packages. Attributes verified by the QC pipe support inspectors included:

locking devices installed and secure

welding complete

prcper material

support configuration

installation clearance guidelines

plumb or cold position

angle of strut / snubber

travel stops in place for spring hangers

clamp and U-bolt condition Subsequently, the NRC inspector conducted field walkdowns of these 15, recently CC verified and accepted, pipe supports. Attributes examined included; location, dimensicnal tolerances, welding, and configuratio The pipe supports were installed in compliance with Sargent and Lundy Specification F/L-2739, M-999 Tolerance drawings, and Phillips-Getschow Company Procedure, QCP-82 (0 pen) Violation (456/84044-07; 457/84040-07): Inspection activities affecting cuality were not effectively implemented in that weld sizes in pipe support / restraints were not identified to te of proper weld

O configurations. To '.arify the licensee's corrective actions the NRC inspector visually inspected 15 pipe supports which had been recently QC verified and accepted. Inspection of the following supports revealed no QC weld inspection deficiencies:

This item will remain open pending the NRC's review of Sargent and Lundy's engineering evaluations of 60 randomly selected pipe supports involving skewed T-fillet weld connections and Phillips-Getschow Company's supplemental weld qualification procedure for skewed fillet welds with skewed angles of 20 and 160 .
