SUBJECT: REPORT NOS. 50-413/88-05 AND 50-414/88-05 Thank you for your responses of March 25, 1988, and March 20, 1988, to our Notice of Violation issued on February 26, 1988, concerning activities conducted at your Catawba facility.
After review of the information provided in your responses to the violation, we accept your violation denial.
Our records will be adjusted.
We understand that a Test Summary will be prepared for each individual actuator following final analysis of the associated data.
The Test Summary will document that actuator springpack adjustment was the method utilized to increase actuator thrust output, and a completed copy of the Test Summary sheet will be filed along with the completed copy of procedure IP/0/A/3820/0?A for each. actuator.
In your response you state that documentation of motor stall test results are available, but not contained with the procedure.
The resulting stall thrust value for each test is recorded and retained by Catawba Technical Support personr.el.
A review of data traces collected with H0 VATS test equipment provides the documentation and verification of steps which were or were not performed and that each completed procedure and associated data traces are reviewed, analyzed and approved by Technical Support personnel. Any misuse of a procedure or failure to perform or adequately document applicable procedure steps would be identified by the absence of required data.
Following this review and approval process the completed package would be fonvarded to Quality Assurance for final review.
You state that on several occasions field personnel were verbally directed to perform the following while accomplishing Procedure IP/0/A/3820/02A:
1.
Measure and record motor stall thrust.
2.
Perform springpack nut adjustment in order to yield the new or desired target thrust.
3.
Minimized durations of stali test.
4.
Not to perform procedural steps when deemed appropriate by Work Supervisor.

We'are concerned with the practice of verbally ~ directing field personnel to supplement on not perform selected procedure steps even though these steps are not required to be signed off.
This subject will be inspected further during a future inspection.
