Licensee Action on Previous Inspection Findings (0 pen) Unresolved Item 78-04-02, Weld to be Radiographed. The a.
Itcensee has reradiographed one of the welds in question (W-14)
and found it to be acceptable. The film was also reviewed by RII and found to be acceptable. The other veld (W-3) is still inaccessible but will be reradiographed when staging is put back in place to complete work in the area. This ites remains open.

Licensee Identified Items (50.55(e))
Prior to this inspection, the licensee had identified several items which were reportable under 10 CFR 50.55(e). The items reported and their status are as follows:
a.
(Open) Item 78-05-01, Rockwell Valve Operator Qualifications.

This item was initially reported by telecon to R11 on May 18, 1978. An interim report was submitted to RII by the licensee in their letter of June 16, 1978. The licensee submitted a supple-mental report to RII in his letter June 16, 1978 describing the investigation underway to assure reliability of the vr2ves in question. The licensee expects to submit a final report by October 1, 1978.
b.
(0 pen) Item 78-14-01, Pipe Whip Rupture Restraints.
On July 18, 1978 the licensee informed RII by telecon of a problem with

fabricated "H" frames sections procurred from their vendor, the Applied Engineering Company. During a visual inspection of the components on site it was noted that double bevel, welds did not meet the full penetration requirements specified. Subsequent inspection showed the over 100 components were deficient in this The licensee has returned the components to the vendor and area.

is conducting surveillance over vendor repair work. The licensee expects to submit a final report by August 17, 1978.
c.
(Open) Item 78-14-02 (Potential 50.55(e) Item) Diesel Generator Fuel Lines. On June 28, 1978 the licensee notified RIl by telecon of a potential problem with their diesel generators.
Their vendor, Colt Industries, informed them that they may have used the wrong size flaring tool for the diesel generator fuel lines, and the fuel lines could be defective. The licensee informed RII that the vendor was expected to be on site during the week of August 7, 1978 to inspect the fuel lines and determine whether there was a problem. After the inspection, the licensee will inform RII of its reportability.

(Open) 78-BU-04 Stem Mounted Limit Switch. The licensees AE (Gilbert Associates) has identified all subject limit switches and solenoid valves and have also contacted various vendors to determine availability of qualified switches and solenoid valves.
Action on this ites has not been completed. The licensee responded to the above bulletin in his letter of April 18, 1978 to RII, in which he presented an interim report. This item remains open pending receipt of the licensees final report.

Contact Mechanism - General Electric Model CR105X.
The licensee response to this Bulletin is contained in his letter of June 12, 1978 to RII. They state that none of the equipment of concern is used in any of the equipment supplied for V.C. Summer Nuclear Station. This ites is closed.
d.
(Closed) 78-BU-06 Defective Cutler-Hammer Type M Relays with DC Coils. The licensees response to this Bulletin is contained in his letter of July 20, 1978 to RII. They found one defective relay in the HVAC control board and have replaced it. This ites is closed.
(Closed) 78-BU-10 Bergen-Paterson Hydraulic Shock Suppressor e.

Exit Interview The inspector met with licensee representatives (denoted in paragraph 1) at the conclusion of the inspection on August 2, 1978.
The inspectors summarized the purpose and scope of the inspection and the review of previous inspection findings. There were no questions or dissenting comments.
