Bolt Hole Size.

Florida Pow'er an'd L'ight Company (FP&L')nformed the'NRC inspector that the bolt hole size. and shape had been evaluated and determined acceptable.

The inspector reviewed FP&L's documentation of the acceptance of the holes in their FCR-2699.

In addition, the inspector examined two of the bolt holes for compliance with the conditions of acceptance stated in FCR-2699.

The NRC inspector reviewed FP&L's requirements for control of cold spring as described in QI 9. 1 Rev.

Potential Unacceptable Safety Injection Pipe Weld.

FP&L informed the NRC inspector that the subject weld had been determined.unacceptable and that their investigation led

to identification of a licensee identified item, as de'cribed in para-graph 5 below.

Unresolved Item 389/80-15-09 is considered a further example of the noncomplying conditions previously identified as infraction 389/80-15-03.

The unresolved item is closed.

. Fa*ilure to Fol;low Radiographic..Procedure On February 12, 1981, FP&L informed Region II tha a deficiency in con-struction had been identified when a

QC inspector had failed to properly follow procedural requirements in review of radiographs of safety related welds.

FP&L stated that the individual inspector had been relieved of all responsibilities for review of radiographs and that assurance had been provided that his replacement was following established procedures.

An independent review of all radiographs interpreted by the subject inspector is being conducted by FP&L.

This item was identified by FP&L as a result of their investigation of an infraction and unresolved item no. 389/80-15-09 disclosed by NRC Region II in a previous inspection ( Infraction 389/80-15-03).

Safety Related Structures (Melding) - Observation of Work and Work Acti-vities The inspector observed welding related activities associated with safety

'related structures and supports'or compliance with code'requirements and.

FSAR commitments.

The code applicable to the areas observed is AWS 01. 1-74.

The activities observed are as follows:

a.

Melding.

Main steam restraint welds 2F-C-STL-G799-060 and 397 were observed in progress for identification/location, proper welding procedure, quali-fied welder, use of specified welding material, and proper NOE speci-fied.

b.

Meld Inspection The NRC inspector observed inspection of the completed welds on safety related piping support SI-2403-540.

The welds were inspected in accordance with the drawing and FP&L procedure QI 9. 1, Rev.

Welding Material Control

The NRC inspector observed welding material controls at the welding material issue station for the above welds for proper storage, temper-ature control, records and handling.

The welding areas were examined for evidence of uncontrolled welding materials.

piping for compliance with NRC requirements and FSAR commitments

Welding Material Control The inspector observed the storage and handling of welding material for weld 2F-C-RC-0123-002 to assure that it was properly controlled, issued, and identified until consumed.

Visual Examination of Welds The NRC inspector observed completed welds 2F-C-RC-0215-013 and 014 for proper location, size, surface finish, and absence of surface defects.

Meld Heat Treatment The NRC inspector reviewed FP&L's preheating requirements and observed a check of preheat on repair weld 2F-C-RC-0123-002 to determine compli-ance with requirements specified (in welding procedure WPS 73RO).

Welder Qualification The NRC inspector. reviewed the performance qualification test -records and welder qualification status report to verify the current quali fi-cations of. the welders who performed the welds identified in 7.a above.

The examination was observed for proper weld surface condition and proper performance of the examination as required by the code for the completed welds.

Within the areas examined, no violations or deviations were identified.

Audits This items addresses concerns regarding the apparently excessive orientation of FP&L audits towards documentation.

The inspector reviewed audit reports and audit checkl'ists for the past two years in areas which. would.be expected to address the proficiency of welding inspection personnel.

"Personnel Welding Control Training Control'of:Speci'alProcess'es'raining Inspection The inspector noted the relative lack of audit checks in which work per-formance was observed to assure maintenance of QC inspector proficiency in required inspections.

As indicated in the original opening of this inspector followup item, emphasis was almost entirely on documentation reviews.

The NRC inspector questioned FP&L QC supervision as to whether QC inspector's work was ever monitored and documented to provide evidence of their proficiency as a basis for required periodic recertifications.

The inspector was informed that such checks were now being conducted.

This item will be examined further in subsequent NRC inspections.
