Thi s IE Circular is closed based on licensee review that indicates that:

the subject model seismic monitor is not used at St.

Lucie Unit 1, and (b) I&E procedure 1400152, Rev.

frequency specified by the Technical Specifications.

Licensee Event Reports Review The following LER's were reviewed to verify that reporting requirements had been met, causes had been identified, corrective actions appeared appro-priate, generic applicability had been considered, and the LER forms were complete.

Additionally, for those reports identified by asterisk, a more detailed review was performed to verify that the licensee had reviewed the events, corrective action had been taken, no unreviewed safety questions were involved, and violations of regulations or Technical Specification conditions had been identified.

Followup of Previously Identified Items (Open)

Licensee Identified Item 78-26-01 (LER 78-25).

This item identified an error in the original analysis of a NSP-4 cask drop for the'pent fuel pool and that the re-analysis required review, prior to acceptance, by the office of Nuclear Reactor Regulation (NRR).

This was submitted by.

FPL letter L-80-342 of 10/16/80 and remains open pending NRR review and acceptanc (Closed) IFI 50-335/79-06-01.

This item concerned modification of procedure AP-0010520 to reflect that the Facility Review Group approves procedures based on unanimous agreement.

The inspector reviewed procedure revision

which included this item in section 8.4.

The inspector had no further questions with respect to this specific item.

A related item concerning the policy of using unanimous agreement is discussed in paragraph 9.

IFI 50-335/79-13-05.

This item referred to changing the plant tagout procedure to provide use of smal,l tags on control room panels to preclude obscuring of indications.

The inspector reviewed procedure OP-0010122, Rev.

12, In Plant Equipment Clearance Orders.

Section 8.3.3.1 gives directions for trimm'ing tags to be placed on any control boards.

The inspector had no further questions on this subject.

As a result of visits to the site and corporate office by various NRC personnel during the months of November and December 1980, a report (PAS 80-4)

was written containing potential violations and "weak" areas.

These were formally identified as Unresolved Noncompliances and Inspector Followup Items in Inspection Reports 81-02 and 81-08.

One additional

"weak" area has been identified as listed below.

(IFI 81-11-01) Facility Review Group uses a unanimous vote for approval of items vice majority rule.

Review of Plant Operation The inspector kept informed on a daily basis of the overall plant status and'ignificant safety matters related to plant operations.

Discussions about planned safety-related activities were held frequently with various members of the plant management and operations staff.

Selected portions of current plant operating logs and data sheets were reviewed during this report period.

The inspector conducted several tours of plant areas.

Observations were made of work in progress, plant housekeeping, and vital area controls.'n Thursday, April 16, 1981 at 3:57 p.m., while operating at 790 MWe Net, a

light rain caused a short to ground around the B phase insulator on breaker 3W in the switchyard.

This ground caused an overcurrent, surge on the B

phase at the output insulator bushing of the main station transformer 1B,

causing the cable to burn in two at the bushing.

The cable hung directly above the separated bushing connection and a large fireball of current was maintained for about 1 minute until plant operators tripped the unit.

All plant systems functioned normally.

The B phase insulators on both main station transformers were replaced prior to restart.

The plant was returned to service at 2:47 p.m. on Tuesday April 21.

No violations or deviations were identified by the inspector during this event.
