10 CFR 50.55(e) Report, dated November 26, 1980 (Nos. 373/80-04 and 374/80-04), Subject: Suppression Pool Water Volume. The inspector reviewed Commonwealth Edison's thirty (30)

day report to RIII dated December 24, 1981, and the final report to RIII dated May 6, 1981, stating that the final Safety Analysis Report was amended (Amendment 54) to reflect the as-built situation.

A re-analysis was performed by General Electric and confirmatory -

re-analysis was performed by Sargent and Lundy showing that a ten percent reduction in suppression pool water volume did not show a marked difference in pool temperature response.. Review of CE's letter to USNRC Licensing Branch dated June 29, 1981 established that Revision 9 to the LaSalle County Station Desi.en Assessment Report included an update of Chapter 6.0 addressimg suppression pool water temperature response.

10 CFR 50.55(e) Report, dated November 12, 1980 (Nos. 373/80-06 and 374/80-06), Subject: Electrical Termination Lug Design. The inspector reviewed the following:

(1) Letter from CE to RIII dated December 15, 1980, Subject: Response to IE Inspection Report No. 50-373/80-43, w/ enclosure.

(2) Letter from RIII to CE, dated January 9, 1981, in response to Item 1 above.

(3) Final report from CE to RIII, dated January 19, 1981, stating that all defective lugs had been replaced.

(4) Review of GE's Field Deviation Disposition Request No. HAl-540 dated September 15, 1978, stating that greater than 200 nr.mber of runs of No. 14 AWG wire terminated with No. 10 or No. 12 ring lugs.

It was noted that travellers were not used to document the lug replacement program; however, FDDR No. HAl-540 indicates disposition completed.

The inspector considers this item closed and in conformance with Item 3 above.

The inspector reviewed S & L letter to CECO (SGE-390) dated April 1,

1981, with enclosures which identified the deficiency ar.d specified corrective action. The inspector considers this item closed and in conformance with CECO's final report to RIII dated April 29, 1981.

The inspector conducted a review of the control of procured material and equipment - receiving and inspection records - and the requirements of CECO Quality Assurance Manual, Quality Pro-cedure Q.P. No. 7-1.

This review was initiated in the receiving area by a review of the receiving log and included the selection of examples of the receipt of safety related items for further detailed analysis. During the initial review the inspector checked the Code Class (1, 2, 3) and the Classification Status (A, B, C) as designated for the 46 MRR's reviewed.

Of particular interest was the Status "B" classification. This

classification is defined as follows in Section 3.1.2 of the CECO Quality Procedure Q.P. No. 7-1:

"3 1 2 Interim Release - (Classification B)

Equipment, components, or materials in an undamaged condition which lack. a required quality documentation may be given interim release on the CECO material receiving report by site QA for installation (not on " Hold") provided the following is received at the site:

a.

For safety-related, non-ASME Section III Code items, an accept-able Certificate of Conformance and, as applicable, either an ASME Code Data Report, Certified Material Test Report or a Material Manufacturer's Certificate of Compliance. Where pro-curement requirements are identical to the ASME Code. require-ments, only the applicable Code document listed above is required to permit interim release of purchased material or items by Quality Assurance for installation.

The requirements for Certificate of Conformance, ASME Code Data Forms and/or Certified Material Test Reports shall be as speci-fied in the procurement documents. Follow-up is required to obtain necessary documentation and close out of the CECO material receiving report (Form QP 7-1.1)."

2/23/77 Hold Tag No. 3751, 2/24/77, issued for dimension problem NRC #145, 2/24/77 - addressed the thin wall question. A G.E. Co. letter of 3/8/77, received 3/10/77, indicated (by calculation) that the minimum wall required was 0.295 inches. Measured wall w,- 0.298 inches which was therefore an acceptable condition. The NRC inspector checked the G.E. Co. calculation for verification and found that the G.E. calculation result of 0.295 inches was in error. The correct result following the logic and data provided by G.E. Co. in their letter is 0.2905 inches minimum wall thickness required. This was reported to the licensee. However, since the actual calculated minimum wall thickness was more conservative than the previously reported thickness by G.E. Co., no further action was required by the NRC inspector. The NRC 145 was signed off 4/19/77. Certificate of conform-ance and copy of ASME data report was reviewed and accepted 4/19/77.

Interim release approved 4/19/77.

" Reducing Type Safe End"..The MRR was accepted by station construction 2/15/79 and by Q.A. on 2/23/79. Final data report review by S & L was completed 6/7/80 and final' acceptance by Q.A. was completed 6/16/80.

cable would be used in safety related applica-tions. A Certificate of Conformance is required and provided for in the purchase order specifica-tion and was assured via telephone communication with supplier 1/29/79. No certificates of con-formance were supplied with the shipment. No hold tags were issued pending followup on 1/29/79 telephone communication. On 4/5/79 additional contact with Rockbestos assured that material was in conformance. On 4/10/79 an interim release was granted by Field Engineering and Q.A., and request to Rockbestos was documented. One reel of wire appears to have been released 5/4/79 for use based on the interim release granted 4/10/79.

On 2/29/80 another letter was sent to Rockbestos regarding the certificates of conformance. No reply received to date, and no certificates received. However, telephone assurances continue.

The status of the Certificate of Conformance for this shipa.ent of instrument cable is still officially known by licensee. This is considered an unresolved item by the NRC inspector (373/81-25-01; 374/81-13-01).

Hold Tags #4913 - 4923 issued 4/1/80 due to status of review of welding procedure. All hold tags were signed off and released upon final review and approval of welding procedure on 4/30/80. An interim release was granted by construction and accepted by Q.A. on 4/1/80 to store and bolt embedments in place pending approval of welding procedure. Standard CBI certifi-cations received with shipment and accepted 4/1/80.

Final record material certifications and test reports were transmitted 10/8/80 by S & L and received by CECO 10/14/80. Final review was completed 1/21/81.

Interim acceptance was approved 7/9/81. No documentation was required other than that furnished previously on original shipment. Final Q.A. acceptance signed off on 7/9/81.

Observations by the Inspector on the Results of the Review The inspector's detailed review of the selected 19 items, out of the original 46 items, indicated that they had been received and processed essentially in accord with the pertinent issue and provisions of the CECO Quality Procedure Q.P. No. 7-1.

Two items, #4 and #9, were found to have had minor problems in their processing after receipt. However, no significant deficienuies developed from these problems. Two items, #13 and #14, irdicate that additional information may be required to establish isn accept-able final status, and to respond to questicas raised by the NRC inspector.

The detailed review tavered eleven (11) specification categories of procurement and is considered to be an adequate representative sample of the procurement activity to demonstrate satisfactory compliance with their program requirements for receiving and inspection, control of procured material and equipment, reporting quality nonconformances, and interim release of components.

Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of non-compliance, or deviations. Unresolved items disclosed during the inspection are discussed in Paragraphs 3.b(13) and 3.b(14).

Exit Interview The inspector met with licensee representatives (denoted under Persons Contacted) at the conclusion of the inspection on July 24, 1981. The inspector summarized the scope and findings of the inspection. The licensee acknowledged the information.
