Inspection Summary Inspection on August'18-21, 1981 (Report No. 50-373/81-31)

Areas Inspected: Routine announced inspection to review licensee action on previous inspection findings in Reports 50-373/78-26 and 50-373/80-55 concerning the implementation of the fire protection and prevention administrative programs and the construction and installation of fire protection equipment and systems. This inspection involved a total of 35 inspector-hours onsite by one region based NRC inspector including six inspector-hours onsite during off-shifts.

Results: Of the two areas inspected, no items of noncompliance were identified in any area.

Licensee Actions on Previous Inspection Findings a.

(Open) Unresolved Item (50-373/78-26-12): Fire Fighting Strategies (Pre-Plans).

The inspector reviewed the applicant's fire fighting strategies which were in the final stages of completion. The content of-the strategies is not consistent with the recommendations in the NRC guidance document, " Nuclear Plant Fire Protection Func-tional Responsibilities, Administrative Controls and Quality Assurance," or_the requirements in Section III.K.12 of Appendix R to 10 CFR 50, " Fire Protection Program for Nuclear Power Facilities Operation Prior to January.1, 1979" as required in-Paragraph 2.C.(27) of the proposed operating license. The applicant will submit a request and justification for exemption from these requirements to the Office of Nuclear Reactor Regula-tion.

When the strategies are completed, the applicant has agreed to place them at the Shift Engineer's Office and at each of the fire brigade equipment cages. The Shift Engineer's copy will remain in the control room area while the copies at the equip-ment cages will be arranged such that they will be brought to

Fire Marshall's office and in the training department for use in training and drills.

brigade in the location and use of the strategies for safety related areas and areas presenting a hazard to safety related equipment and cabling.

(Open) Unresolved Item (50-373/80-55-01): Fire Protection and Prevention Administrative Controls.

(1) The inspector reviewed the applicant's administrative controls and found that the listed changes must be made to the following procedures to satisfy the fire protection program requirements:

(a) LAP 900-14 Fire Protection Program 1.

Fire Brigade Training - This section will be upgraded to include a requirement that each fire brigade member attend annual hands-on live-fire practice sessions designed to provide training in utilization of the plant fire protection equipment and self-contained breathing apparatus in fighting fires similar in nature and complexity to those which could occur at the plant.

Additionally, each fire brigade member will attend smokehouse training every two years to simulate use of self-contained breathing apparatus in a fire environment.

All fire brigade training will be completed prior to fuel load.

2.

Fire Brigade Qualifications - This section will-reflect the above mentioned changes in the fire brigade training requirements.

3.

Off Site Fire Department - This section will be modified to require that training be provided to the Off Site Fire Department on an annual basis in radiation safety, plant layout and operation, and security and fire protection systems and equipment.

4.

Penetration Fire Barriers Fire doors will be added to the list of penetration fire barriers.

5.

Contractor Training - Administrative controls of combustible and flammable material and ignition source use will be included in the contractor indoctrination training.

(b) LAP 900-15 Housekeeping A requirement for weekly fire hazard inspections of all safety related areas and areas presenting a hazard to safety related equipment and cabling will be included in this procedure.

(c) LAP 900-10 Fire Prevention for Welding and Cutting 1.

This procedure will be upgraded to include controls over flammable gasses, prohibiting their storage in safety related areas and areas presenting a hazard to safety related equipment or cabling, and controls over weld rod ovens to protect safety related equipment and cabling from damage.

The effective time period for Cutting and Welding Permits will be limited to 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> and permits for ignition source work in the Control Room, Cable Spreading Room and Auxiliary Electric Equip-ment Room will require Shift Engineer approval before work may begin.

(d) LAP 900-17 Fire Drills 1.

The frequency of fire drills will be increased to provide drills at regular intervals not to exceed three months for each shift fire brigade.

At least one drill per year for each shift fire brigade will be on a shift other than normal day shift. Each fire brigade member should participate in each drill, but must participate in at least two drills per year.

The fire fighting strategies will be utilized and reviewed during fire drills.

3.

Fire drills will be critiqued on a three year interval by qualified individuals independent of Commonwealth Edison Coapany staff.

(2) The inspector reviewed the fire protection training lesson plans for general employees, contractors and fire brigade members. One apparent deficiency was in Lesson Plan 4-LPAR NGET/ Annual Retraining - Safety. This lesson plan will be upgraded to include training in station operating administrative controls of combustible and flammable materials and ignition and heat sources.

o The requirements in Section III.I of Appendix R to 10 CFR 50 concerning fire brigade training are not satisfied by the applicant's training program as required in Paragraph 2.C.(27) of the proposed operating license.

The applicant will submit a request and justification for exemption from these requirements to the Office of Nuclear Reactor Regulation.

(3) The applicant's proposed program for supervision and surveillance of fire doors does not comply with Section III.N of Appendix R to 10 CFR 50 as required in Paragraph 2.C.(27) of the proposed operating license. The applicant will submit a request and justification for exemption from these requirements to the Office of Nuclear Reactor Regulation.

(4) The applicant's proposed program for fire hose hydrostatic testing does not comply with Section III,E of Appendix R to 10 CFR 50 as required in Paragraph 2.C.(27) of the proposed operating license. The applicant's proposed program is consistent with the current industry standards for fire hose hydrostatic testing, NFPA 1962-1979, " Care, Use and Maintenance of Fire Hose Including Connections and Nozzles." The applicant will submit a request and justification for exemption from these requirements to the Office of Nuclear Reactor Regulation.

and Systems (1) The applicant's proposed self-contained breathing apparatus for the fire brigade does not comply with Section III.H.

of the proposed operating license. The applicant will subett a request and justification for exemption from these requirements to the Office of Nuclear Reactor Regulation.

(2) The inspector will verify complete installation and adequate operation of the fire protection and detection systems and equipment in accordance with the approved fire protection plan and Section 9.5 of the Safety Evaluation Report through review of the completed fire protection and detection system demonstration, SD-FP-101, and direct inspection of installed equipment after the system demonstration has been completed.

3.

Exit Interview The inspector met with licensee representatives denoted in Paragraph 1 at the conclusion of the inspection on August 21, 1981. The inspector reviewed the purpose and scope of the inspection and summarized the findings.
