Fire Protection / Prevention Commitments and Modifications (Unit 1)
The inspector evaluated the licensee's action on the-fire protection commitments made to the NRC. The NRC Fire Protection Safety Evaluation Report (FPSER), Amendment 11 to operating license and the licensee's Farley Nuclear Plant Fire Protection Program Reevaluation (FPPR) dated September 15',1977 with Amendments 1 through 4 were used in this evaluation.

(7) The automatic sprinkler systems contain a number cf discrepancies from the provisions of NFPA-13. The licensee has identified these discrepancies and. is currently evaluating Unit I for conformance to 10 CFR. 50, Appendix R, Section III.G, Fire Protection of Safe Shutdown Capability.
It appears that additional automatic

rivet or screws spaced no more than 8 inches apart on both sides

of the fire damper frame. The above dampers were attached to the duct by rivets only on one side of the damper frame. Also, the

multi-section fire dampers are required by the manufacturer to be i
connected by tack welds, rivets, or screws on each side of the l
damper frame. The installed multi-section dampers were attached by rivets only on one' side of the damper frame. These dampers had been inspected and approved by the licensee's (contractor) QC
inspector.
However, it appears that neither the installation personnel nor the QC inspection personnel were provided with I
-
sufficient documents to assure that the dampers were p.roperly installed _ FPPR Section 4.4.4.1.10 (page 4 - 376) states that the modifications to the fire barrier penetrations including door and duct penetrations will achieve compliance to the NRC guidelines of Appendix A to BPT 9.5.1.
Appendix A, Section D.1(j) requires ventilation systems to be provided with " fire door dampers"

NFPA-80, Section 2-89 requires all fire door components to be installed in accordance with the manufacturer's installation instructions.
The sub-standard fire damper installation is a failure to meet a commit-
.
ment to the NRC and is identified as Deviation Item (348/81-06-01). The licensee has. agreed to reinspect all fire dampers in both Units 1 and-2 to-assure that the fire dampers are properly-attached to the ducts, multi-section dampers are proparly connected and the ducts are properly attached at each barrier penetration.

(122) The licensee is reevaluating the fire resistant rating of approximately 20 doors located in various locations throughout the plant. Many of these doors were installed to meet a combination of either security and fire or watertight and fire criteria and do not appear to meet the required fire resistant rating as committed to the NRC. The license's evaluation will determine which doors need to be replaced or which commitments must be revised. This item is identified as ihresolved Item (348/81-06-02), Licensee's evaluation of fire re Mstant rating of fire barrier doors, and will be reviewed durir; a subsequent NRC inspection.

Fire Protection System Inspection and Tests The following inspection and test data. was. reviewed and discrepancies noted:

(1). FNP-1-EPT-174, Functional Test for Fire Detection System Additions
..iis test was completed on March 1, 1981. However, the tests did not test any of the-fire detectors. The licensee could not locate
the vendor's test documents for the detector units.
However, to meet-the provisions of.the technical specifications, the licensee i
plans to verify the operability of each fire detector unit before Unit 1 is returned to power.
