As a result of the inspection conducted on March 10-13, 1981, certain of your activities appear to deviate from your e7mmitments to the Commission as indicated
-below:
Section 4.4.4.1.10 of the Farley Nuclear - Plant Fire Protection Program Reevaluation (FPPR) dated Septer6er 15,1977 with Amendments 1 through 4 (Amer:dmen*e 4 dated. January 3,1979) states that modifications will be made to the ventilation duct penetrat'ons of fire barriers to achieve compliance with tha NRC guidelines of Appendix-A to Branch Technical Position 9.5.1, Guidel'nes for Fire Protection for Nuclear Power Plants Docketed Prior to July 1, 1976. Appendix A Section D.1(j) requires ventilation duct systems to be provided with " fire door dampers" installed in accordance with National Fire Protection Association Star.dard No. 30 (NFPA-80), Fire Doors and Windows, at all locations where the ducts penetrate fi e barriers.
NFPA-80 Section 2-8.9 requires fire door assemblies to be installed in accordance with the manufacturer's installation instructions.
Contrary to the above, the fire damper was not installed in accordance with the manufacturer's installation istructions.
